摘　　要：目的:通过对指定病种的费用分析比较,探讨临床路径对鞘膜积液手术治疗病种费用的作用,为解决"看病贵"提供切实可行的科学依据。方法:采用回顾性研究和多元回归分析,对82例鞘膜积液手术治疗患者的年龄、住院天数、检查项目、治疗措施、手术时间、手术方式等方面进行筛选,从而找出临床路径能合理、有效降低患者费用的原因。结果:临床路径组和对照组患者的费用在总费用[(4 167.49±1 037.90)、(4 865.26±1 171.53)元]、药品费用[(1 234.27±628.03)、(1 684.56±928.24)元]、检查治疗费用[(1 295.22±434.43)、(1 524.80±400.10)元]、住院天数[(4.88±0.33)、(9.14±1.54)d]方面比较,差异有统计学意义(t=2.57,P<0.05;t=2.21,P<0.05;t=2.38,P<0.05;t=13.65,P<0.001)。结论:临床路径有效规范了医疗行为,合理缩短了医治疗程,合理配置了医疗资源,切实降低了病种费用,真实提高了患者满意度。Objective： To explore the effect of clinical path in the cost of hydrocele surgery,and to provide practical scientific basis in order to address the ＂expensive＂ by analyzing and comparing the cost of specific disease.Methods： A retrospective study and multiple regression analysis were taken in the age,length of stay,inspection items,treatment measures,operative time,surgical and other aspects of screening of the 82 cases of hydrocele surgery patients,and to identify the reason that the clinical pathway can be reasonably and effectively reduce the cost of the reasons for the patient.Results： There were significant differences between the clinical pathway group and the cost of the control group of patients in the total cost [（4 167.49±1 037.90） yuan,（4 865.26±1 171.53）yuan],drug costs [（1 234.27±628.03）yuan,（1 684.56±928.24）],examination and treatment costs [（1 295.22±434.43） yuan,（1 524.80±400.10） yuan] and hospital stay [（4.88±0.33） d,（9.14±1.54） d）（t=2.57,P0.05;t=2.21,P0.05;t=2.38,P0.05;t=13.65,P0.001）.Conclusion： The clinical pathway to effectively regulate the medical practices,shortened the course of treatment and reasonable,rational allocation of medical resources,and effectively reduce the cost of disease,and the real increase patient satisfaction.
