摘  要:目的:通过观察针刀松解干预后上位中枢不同部位β-内腓肽(β-EP)的变化并与电针干预相比较,探讨针刀松解在脊髓以上水平的镇痛机制。方法:将60只健康SD大鼠随机分为空白组、模型组、电针组和针刀组。将4%木瓜蛋白酶溶液与0.3mol/L半胱氨酸溶液按1:1混合静置0.5h后分别于造模第1、4、7天注射于模型组、电针组和针刀组大鼠左膝关节腔,每次20μL。4周后针刀组和电针组给予相应治疗。治疗3周后大鼠断头处死,取出脑组织,分别测定延髓、丘脑、下丘脑、垂体、海马的β-EP含量,针刀组分别与空白组、模型组、电针组进行对比。结果:在膝骨关节炎形成过程中,模型组脑内各不同部位的β-EP含量均呈现出升高的趋势,与空白组相比差异有统计学意义(P<0.05,P<0.01)。在针刀及电针干预后β-EP含量在多数部位均表现出下降的趋势,其中在丘脑水平针刀组、电针组的β-EP含量比模型组明显降低,有显著性差异(P<0.05);在丘脑水平2个干预组的β-EP含量更为接近空白组。除在海马水平针刀组与电针组存在显著差异外,在脑内其余各水平针刀组与电针组的β-EP含量差异并不明显。结论:针刀松解法对膝骨关节炎大鼠中枢各水平的β-EP有良性调节作用,这种调节可能与膝关节局部伤害性刺激减弱有关。Objective: To study the analgesia mechanism of acupotomy lysis in brain by comparing the changes of β-EP content in different parts of brain of rats that have undergone acupotomy lysis and electro-acupuncture respectively.Method:Divide 60 healthy SD rats randomly into normal control group,model group,electro-acupuncture(EA) group,and acupotomy lysis(AL) group.Mix 4% papain solution with 0.3mol/L cysteine solution in the ratio of 1:1.Pause for 0.5h,inject the mixture,20μL each time,into the left knee joint cavities of rats in model,AL,EA groups on the frist fourth seventh day,4 weeks after treat AL group with acupotomy lysis and EA group with electro-acupuncture.Three weeks after treatment,take samples of medulla oblongata,thalamus,hypothalamus,pituitary gland,and hippocampus from rat brain.Measure separately the content of β-EP therein.Compare the β-EP content of acupotomy lysis group respectively with that of normal control group,model group,and electro-acupuncture group.Result: during the formation of KOA,the β-EP content in different parts of brain of model group rats all show a tendency to rise,a clear difference compared with normal control group(P〈0.05,P〈0.01).After acupotomy lysis or electro-acupuncture interference,the β-EP content in most parts of brain show a tendency to reduce,especially in thalamus,where β-EP content of acupotomy lysis or electro-acupuncture group is conspicuously lower than model group(P〈0.05).The thalamus β-EP content of acupotomy lysis and electro-acupuncture groups are close to that of normal control group.No distinct difference in β-EP content in different part of brain is found between acupotomy lysis group and electro-acupuncture group,except in hippocampus.Conclusion: Acupotomy lysis functions to regulate in a positive manner the β-EP content in different parts of central nervous system of rats with KOA,the possible mechanism may relation to the weaken of noxious stimuli.
