摘  要:[目的]研究婴儿巨细胞病毒感染(HCMV)的免疫功能变化.[方法]以32例HCMV潜伏性感染患儿(潜伏组),92例活动性HCMV感染患儿(活动组)及48例健康婴儿(正常组)为研究对象;采用散射比浊法检测血清免疫球蛋白和补体,采用流式细胞仪检测法检测患儿外周血T、B细胞亚群和NK细胞.[结果]三组间B细胞、IgG、IgM、IgA、IgE、NK细胞、C3、C4差异无统计学意义(P〉0.05);HCMV感染组总T细胞及CD4+降低,与正常对照组差异有统计学意义(P〈0.05、P〈0.01);正常组和潜伏组的CD8+细胞水平显著低于活动组(P〈0.01),而正常组和潜伏组CD8+细胞水平差异无统计学意义(P〉0.05);CD4+/CD8+水平依次正常组显著高于潜伏组,潜伏组显著高于活动组(P均 〈0.01).[结论]HCMV感染机体后对患儿的天然免疫中的NK细胞、C3、C4以及体液免疫无影响,而细胞免疫功能有明显异常.[Objective]To investigate the change of immunologic function in infants with human Cytomegalovirus(HCMV) infection. [Methods]A total of 32 infants with latent HCMV infection(latent group), 92 infants with active HCMV infection(active group) and 48 healthy infants(normal group) were enrolled in this study. Nephelometry was used to detect serum immunoglobulin and complement. Peripheral blood T and 13 cell subsets and NK cells were determined by flow cytometry. [Results] There was no significant difference in Blymphocyte, IgG, IgA, IgM, IgE, NK cells, C3 and C4 among three groups( P 〉0.05). Total T-lymphocyte and CD4+ decreased in HCMV infection groups, and there was significant difference between HCMV infection groups and normal group( P 〈0.05, P 〈0.01). The level of CD8+ in normal group and latent group was obviously lower than that in active group( P 〈0.01), but there was no significant difference between normal group and latent group( P 〉0.05). The CD4+/CD8+ level in normal group was obviously higher than that in latent group, and CD4+/CD8+ level in latent group was significantly higher than that in active group(all P 〈 0.01). [Conclusion] HCMV infection has no influence on NK cells, C3, C4 and humoral immunity of infants. However the cellular immunity has obvious normal in infants with HCMV infection.
