多排螺旋CT三维重建在腕、踝关节损伤中的诊断价值

摘　　要：目的评价多排螺旋CT[MDCT]三维重建技术在诊断腕、踝关节损伤的诊断价值。方法收集40例腕、踝关节临床考虑骨折的外伤患者行螺旋CT扫描,并在工作站上进行多平面重建(MPR)、曲面重建(CPR)、表面遮盖法(SSD)及容积再现(VR)后处理重组,由2名有经验的放射科医生和2名骨科医生对后处理图像进行观察,并与X线片结果对比分析。结果 CT发现骨折40例,X线发现骨折35例,X线共发现骨折40处,MPR、VP共发现51处。10例涉及关节面的骨折,VR、MPR显示最佳,清晰显示骨折全貌。SSD很好的显示骨折脱位周围软组织、皮肤表面血管。6例石膏外固定患者运用MIP成像,对比平片得到更为清晰的三维图像。结论多排螺旋CT三维重建技术在腕、踝关节损伤具有很高的诊断价值。Objective To evaluate the multidetector-row CT [MDCT] three-dimensional reconstruction technique in diagnosis of wrist, ankle injury diagnostic value. Methods Collecting 40 patients with clinical consider wrist, ankle fractures of the trauma patients were done a spiral CT scan, and more at the work station (MPR), planar reconstruction surface reconstruction (CPR), surface cover method (SSD) and cubage emersion (VR) post-processing reorganization, two experienced by radiologists and two orthopedic surgeon carries on the observation of post-processing of images, and the results were compared with X-ray analysis. Results CT found fractures in 40 cases, X-ray found fractures, 35 patients were found fracture of X line MPR, VP at 40, were found in 51,10 cases involving articular surfaces, VR, MPR fractures and fractures showed clearly show the best picture. SSD good display fracture dislocation of soft tissue, skin surface around the blood vessels. Six cases of gesso external fixation using MIP imaging, compared with radiographs gets more clear 3d images. Conclusion Multidetector-row CT 3d reconstruction technique in the wrist, ankle injury has high diagnostic value.
