3R护理改善阿尔茨海默病患者生活质量的研究

摘　　要：目的通过神经心理学指标评定，系统评价Reminiscence（往事回忆），Reality Orientation（现实定向）和Remotivation（再激发）的3R护理疗法对改善阿尔茨海默病患者生活质量的效果。方法本研究采用实验型研究设计，选取阿尔茨海默病患者34例，进行神经心理学指标fMMSE、ADL、FAQ、NPI）的基线评定，然后随机分组，对照组给予常规治疗，治疗组在常规治疗基础上采用3R疗法进行认知功能训练。在开始实施训练后的第3个月、第6个月时，分别对试验组和对照组的患者进行各项指标的评定与分析。结果（1）3R治疗组6个月的ADL评分（31．02±2．01），较基线值（37．41±4．24）明显下降，提示生活能力显著提高（P〈0．01）;（2）3R治疗组治疗后FAQ评分（15．12±1．09）较治疗前（15．99±2．35）减低，提示治疗后社会活动能力提高沪〈0．05）;（3）3R治疗组治疗后NPI评分（10．11±2．03）较治疗前（13．5±1．14）下降，表明行为过激发生率降低垆〈0．01）。结论3R治疗能有效提高阿尔茨海默病患者生活自理能力、社会活动能力，改善患者及照料者生活质量。Objective Index by neuropsychological assessment, systematic reviews Reminiscence, Reality Orientation, and Remotivation of the 3R care therapy to improve the quality of life of patients with Alzheimer＇s effects.Methods In this study, experimental study design, selected 34 cases of Alzheimer＇s disease, neuropsychological indicators （MMSE, ADL, FAQ, NPI） of the baseline assessment, and then randomized to the control group received conventional therapy, the treatment group were 3R therapy treatment based on the use of the cognitive training. After training at the beginning of the implementation of the first 3 months, 6 months, respectively, the experimental group and control group of patients and analysis of the assessment of the indicators.Results （1） 3R treatment group significantly increased viability （P 〈 0.01）; （2） 3R improve social skills therapy group （P 〈 0.05）; （3） 3R behaving aggressively treated group reduced the incidence （P〈 0.01）.Conclusions 3R treatment of Alzheimer＇s disease can effectively improve the daily living, social skills, improve quality of life for patients and caregivers.
