腹腔镜在未婚女性巨大卵巢囊肿中的应用

摘　　要：目的探讨未婚女性巨大卵巢囊肿行腹腔镜手术的方法及其安全性、依从性。方法对21例未婚巨大卵巢囊肿患者行腹腔镜辅助微小切口治疗,镜下行囊肿穿刺抽吸囊液,经2～3 cm Trocar切口将囊肿拉出腹壁外剥除,并行卵巢成形术送回腹腔。结果 21例手术均在腹腔镜辅助微小切口下顺利完成,术后病理均为良性,平均手术时间(56±13.1)min,平均出血量(40±8.4)ml,囊肿平均直径24.8 cm,吸出囊内液平均4 800 ml。术后随访囊肿无复发。术后平均住院5 d。结论腹腔镜辅助微小切口行巨大卵巢囊肿手术具有安全、有效、创伤小的优点,尤其能提高未婚年轻女性生理和心理依从性,值得临床推广。Objective To investigate the safety,feasibility and scientific of Laparoscopic-assisted small incision surgery for the treatment of huge oophoritic cyst among unmarried young women.Methods 21 cases of huge oophoritic cyst were treated laparoscopically assisted small incision,paracentesis were performed under direct vision.The cysts walls were taken out through the 2cm^3cm Trocar incision for treatment.Then return to abdominal cavity after oophoroplasty.Results 21 cases were treated by Laparoscopic-assisted small incision successfully.Pathology was benign.The mean operation time was（56±13.1min）;the mean blood loss was（40±8.4ml）;the mean cyst diameter was 24.8 cm;the mean Cyst fluid was 4800 ml.No complication happened in all patients.No recurrence after postoperative.Mean postoperative hospital stay was 5d.Conclusion It is safe,effective and minor trauma to manage huge oophoritic cyst by laparoscopy.In particular,it is important for unmarried young women,as well as fertility requirements.
