经脐单孔腹腔镜胆囊切除术20例报告

摘　　要：目的:探讨经脐单孔腹腔镜手术的应用价值。方法:回顾分析2010年7月至2011年1月为20例患者行经脐单孔腹腔镜手术的临床资料。结果:18例行胆囊切除术,2例同时切除胆囊及阑尾,手术均获成功,无中转开腹或常规腹腔镜手术,手术时间15～70min,术中出血少,均未放置引流管,无并发症发生,术后2～4d出院。随访1个月腹部瘢痕不明显。结论:经脐单孔腹腔镜胆囊及阑尾切除术安全可行,比常规腹腔镜手术更微创,具有患者疼痛轻、腹壁瘢痕不明显等优点。但手术操作相对困难,对术者技术水平要求高,可在有条件的医院选择适合病例开展。Objective：To discuss the clinical application of transumbilieal single port laparoseopie surgery. Methods：The clinical data of 20 patients with gallbladder and appendix diseases undergoing transumbilical single port laparoscopie surgery from Jul. 2010 to Jan. 2011 were analyzed retrospeetively. Results： All the operations were successfully performed. 18 patients underwenl eholeeystectamy ,2 patients aeeepted eholeeystectomy and appendeetomy. None of them experieneed conversion to open surgery or conventional laparoseople surgery. The operating time ranged from 15 to 70 min, the blood loss was little without drainage tube. No complleations oceurred,postoperative hospilal stay was 2-4 （1. The abdominal wall sear was not obvious in follow-up of one month. Conclusions：Transumhilieal single port laparoscopie eholeeysteetomy and appendectomy is safe and feasihle. Compared with the routing laparoseopic surgery,it has more mini-invasive advantages, less pain and ahdominal wall sear,hut the operation is difficult to perform. This surgery can he applied in qualified hospital with suitahle indication.
