摘  要:目的探讨超声弹性成像定量分析在甲状腺结节性病变良恶性诊断中的应用价值。方法对167例患者的212个结节作超声弹性成像检查,分析结节的应变率和应变率比值与病理结果间的关系。结果恶性组与良性组的应变率和应变率比值的比较,差异有统计学意义(t分别=9.67、9.03,P均<0.05)。甲状腺结节应变率的良恶性分界点约为0.16%,以此应变率标准判断结节良恶性,敏感性为77.90%、特异性为88.90%、准确性为84.90%。应变率比值的良恶性分界点约3.14,以应变率比值判断良恶性,敏感性为80.50%、特异性为90.40%、准确性为86.80%。结论超声弹性成像定量分析法可以用来评估甲状腺结节的良恶性,且准确性较高。Objective To explore the application values of elastosonographic quantitative analysis of thyroid nodules in diagnosing benign and malignant disease.Methods One hundred and sixty-seven patients with 212 thyroid nodules were examined by elastosonography,and compared with the pathological results.Results The ratio of strain rate and strain rate had statistically significance between the malignant and benign group(t=9.67,9.03,P0.05).Strain rate of thyroid benign and malignant nodules cut-off point was about 0.16%,as the standard to determine the strain rate of benign and malignant nodules,sensitivity was 77.90%,specificity was 88.90%,accuracy 84.90%.Malignant and benign strain rate ratio of about 3.14 cut-off point to determine the strain rate ratio of benign and malignant,the sensitivity was 80.50%,specificity 90.40%,accuracy 86.80%.Conclusions Elastosonographic quantitative analysis of benign and malignant thyroid nodules is intuitive and accurate.
