高龄髋部骨折患者术前心理状态与手术风险的相关性研究

摘　　要：目的探讨高龄髋部骨折患者术前心理状态与手术风险的相关性,为高龄髋部骨折患者术前心理干预提供理论依据。方法通过对90例符合纳入与排除标准的高龄髋部骨折患者进行术前认知功能,抑郁和焦虑心理状态测评,根据测评结果按相关标准将每种心理状态分为正常、轻度与重度组,以术中至术后3个月内并发症发生率作为手术风险高低的指标,比较不同心理状态下手术风险的差异,统计术前心理状态与手术风险的相关性。结果不同认知功能组患者术中至术后3个月内并发症发生率差异无统计学意义(P>0.05),不同抑郁与焦虑组术中至术后3个月内并发症发生率差异有统计学意义(P<0.01),术中至术后3个月内并发症的发生与抑郁心理和焦虑心理呈正相关。结论高龄髋部骨折患者术前抑郁和焦虑心理状态与手术风险高度相关,抑郁是影响手术风险的最主要心理因素,对患者术前行抑郁和焦虑方面的心理干预是非常必要的。Objective To explore the relationship between preoperative psychological states and operational risk in elderly patients with hip fracture, and to provide theoretic reference for preoperative psychological intervention. Methods 90 elderly patients with hip fracture were tested for their preoperative cognition function, depression and anxiety, the results of which were taken as the major principle to divide the patients into cognition group, anxiety group and depression group in accordance with related standards. The recent complication incidence in operation and within 3 months after operation was taken as an index of operational risk. Operational risks of patients in different groups were compared so as to find out the relationship between preoperative psychological states and operational risk. Results There were no obvious differences of the recent complication incidence in cognition group （P〉0.05）, there were, however, obvious differences of the recent complication incidence rates in depression and anxiety group （P〈0.01）. There was positive correlation between the recent complication incidence, depression and anxiety. Conclusion There is significant correlation between preoperative depression, anxiety and operational risk in aged patients with hip fracture, and depression exerts great influence on operational risk. It is necessary to run intervention on preoperative depression and anxiety in aged patients with hip fracture.
