中药分期治疗对子宫腺肌病痛经6-k-PGF_(1α)、PGE_2影响的观察

摘　　要：目的:观察活血消癥胶囊合化瘀止痛胶囊分期治疗子宫腺肌病痛经的临床疗效及作用机理。方法:将65例以痛经为主诉的子宫腺肌病患者随机分为治疗组35例(口服活血消癥胶囊与化瘀止痛胶囊)和对照组30例(口服桂枝茯苓胶囊与元胡止痛胶囊),比较两组痛经、子宫大小、血清6-k-PGF1α、PGE2、EmAb的变化。结果:治疗后两组痛经症状明显改善,两组疗效差异有非常显著性(P<0.01);治疗后两组子宫体积明显缩小、血清6-k-PGF1α、PGE2值均明显降低,组间比较差异有显著性(P<0.05或P<0.01);两组疗后血清EmAb阳性率降低,但组间比较无显著性差异(P>0.05)。结论:活血消癥胶囊合化瘀止痛胶囊分期治疗子宫腺肌病能缓解痛经症状、缩小增大的子宫,可能与降低了PGE2含量,从而减轻了因增生和炎症带来的疼痛症状,促进了慢性炎症和免疫复合物的吸收,提高了机体的免疫力有关。Objective：To observe the clinical effect and mechanism of treating adenomyosis dysmenorrhea with Huoxue Xiaozheng capsules and Huayu zhitong capsules by stages.Methods：65 patients of adenomyosis dysmenorrhea were randomly divided into two groups,in which,35 cases in treatment group using HuoxueXiaozheng capsules and Huayu Zhitong capsules by stages,and 30 cases in control group using Guizhi Fuling capsules and Yuanhu Zhitong capsules by stages,then compared the changes of dysmenorrhea symptoms, uterine volume,blood prostaglandin6-k-PGF1α,prostaglandin E2 and EMAb.Results：The dysmenorrhea symptoms of the two groups were obviously improved after treatment and there were significant differences between 2 groups（P〈0.01）.Uterine volume,level of blood 6-k-PGF1αand PGE2 in 2 groups were all obviously decreased after the treatment,which had significantly difference（P〈0.05 or P〈0.01）; The positive rate of EMAb of patients in two groups were decreased after the treatment,but which had no significant differences（P〈0.05）. Conclusion：Treating adenomyosis dysmenorrhea with Huoxue Xiaozheng capsules and Huayu Zhitong capsules by stages can ease symptoms of dysmenorrhea,reduce the increased uterine volume,and decreased the level of PGE2,so that reduced the dysmenorrhea symptoms caused by inflammatory exudation and hyperplasia,and promoted the treatment of chronic inflammation and assimilated of immune complex,then improved the body’s immunity.
