改良悬雍垂腭咽成形术加舌根等离子消融术的临床疗效

摘　　要：目的探讨改良悬雍垂腭咽成形术(Hanuvulopalatopharyngoplasty,H-UPPP)加舌根等离子消融术治疗阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的疗效。方法本组中重度OSAHS患者88例,采用H-UPPP加舌根等离子消融术,术后6个月及术后1年多道睡眠图监测随访。结果 88例患者,术后6个月综合疗效评价:治愈4例,显效65例,有效15例,无效4例;术后1年综合疗效评价:治愈2例,显效62例,有效18例,无效6例。结论 H-UPPP加舌根等离子消融术,不仅缓解了患者的阻塞症状,而且对于并发症亦有明显疗效,手术安全、有效,术后反应轻,复发率低,值得推广。OBJECTIVE To evaluate the effect of combination of tongue base radiofrequency ablation and Han-uvulopalatopharyngoplasty（H-UPPP） in the treatment of OSAHS.METHODS Eighty-eight patients with severe OSAHS were included in this research.Each patient was performed with operations including both radiofrequency ablation in tongue base and H-UPPP.All patients were followed-up after operation.Polysomnogram（PSG）was performed on each patient at the 6th and 12th month after operation respectively.RESULTSTherapeutic effect was evaluated by using standard of Hangzhou meeting（2002）.Six months after operation,four patients were cured,sixty-five patients were markedly improved,fifteen patients were improved,and 4 patients were not improved.One year after operation,two patients were cured,sixty-two patients were markedly improved,eighteen patients were improved,and 6 patients were not improved.CONCLUSION The combination of tongue base radiofrequency ablation and H-UPPP can not only release the symptoms of obstruction,but also have effect on postoperative complications.It＇s a safe,effective procedure,with mild postoperative reaction and low recurrence rate.And it deserves to spread.
