不同麻醉方式对妇科无气腹腹腔镜手术操作空间的影响

摘　　要：目的观察硬膜外麻醉及腰硬联合麻醉在妇科无气腹腹腔镜手术中对手术操作空间及安全性的影响。方法回顾性分析2009年5月—2010年8月在河北医科大学沧州中西医结合医院完成的妇科无气腹腹腔镜手术患者38例的临床资料。硬膜外麻醉16例,腰硬联合麻醉22例。结果 38例手术3例中转气腹手术。硬膜外组麻醉术中管理较为困难,肌肉松弛效果相对欠佳,腰硬联合麻醉组麻醉相对满意。结论腰硬联合麻醉具有起效快、阻滞广泛、肌肉松弛效果好、麻醉可控性强等特点,更适用于妇科无气腹腹腔镜手术麻醉。Objective To observe the effect on operation space and safety between epidural anesthesia （EA） and combined spinal epidural anesthesia （CSEA） in gynecological gasless laparoscopic surgery. Methods A retrospective analysis of clinical data on 38 cases of gynecological gasless laparoscopic surgery from May 2009 to August 2010 in Hebei Medical University Affiliated Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine. EA group had 16 patients,while CSEA group 22 patients. Results Among 38 cases of gynecological gasless laparoscopic surgery,3 cases were converted to common gynecologic laparoscopic surgery. Patients of EA group were more difficult to manage in operation with poorer muscle relaxation effect than CSEA group;CSEA group were more satisfactory in anesthesia than EA group. Conclusion Combined spinal epidural anesthesia with rapid onset, widespread block, notable muscle relaxation effect, strong controllability are more suitable for gynecological gasless laparoscopic surgery.
