单操作孔胸腔镜治疗原发性自发性气胸32例临床体会

摘　　要：目的探讨单操作孔电视胸腔镜（VATS）治疗原发性白发性气胸（PSP）的可行性和疗效。方法凹顾分析2009年9月至2010年12月行单操作孔VATS治疗32例PSP患者的资料。操作孔位于腋前线3～4肋间，切口长1．5—20cm，观察孔位于腋中线7～8肋间，切口长1cm。结果3l例经单操作孔VATS顺利完成手术，1例术中中转为传统VATS。手术时间30～100min，平均50min；术中出血量20～120ml，平均45ml；胸管引流量30～100ml，平均55ml；住院时间2～5d，平均3．5d。全部顺利出院，无并发症。随访5～21个月，平均14个月，无复发及慢性疼痛。结论单操作孔VATS治疗PSP是安全可行的，能减少手术创伤且疗效理想。Objective To discuss the feasibility and advantages of uniportal video - assisted tho- racoscopic surgery（UVATS） in the treatment of primary spontaneous pneumothorax（PSP）. Method From September 2009 to December 2010,32 patients with PSP received UVATS, and their clinical data were restrospectively analyzed. The utility port for operation（ 1.5 - 2.0 cm long） was located at the third or fourth rib in the anterior axillary line,while the port for observation（ 1.0 cm long）was located at the seventh or eighth rib in the middle axillary line. Result There were 31 patients subjected to UVATS suceesfully,and 1 patient was converted to traditional video - assisted thoracoscopic surgery. Operative duration was 30 - 100 min, averaging 50min ; intraoperative blood loss was 20 - 120 ml, averaging 45 ml ; chest drainage was 30- 100 ml, averaging 55ml; Hospital stay was 2 -5 days, averaging 3.5 days. All patients were discharged without severe complications. During a follow -up for 5 -21 months （averaging 14 months）, there were no recurrences or chronic pains. Conclusion The UVATS has advantages of minimal invasion and good curative effect,which is a safe and feasible treatment for PSP.
