摘　　要：目的:为了探讨急性心力衰竭(AHF)患者治疗前后血脂和脑钠素(BNP)测定的临床意义。方法:108例AHF患者(包括治疗后好转的62例和恶化的30例)及60例正常对照组采用生化法测定了血脂水平(TC、TG、HDL-C和LDL-C),化学发光免疫分析测定了BNP水平,并进行了对比性分析。结果:108例AHF患者的血脂分析表明,血清TC、TG、HDL-C和LDL-C水平较60例正常对照组均明显降低(P均<0.01);62例AHF患者的血脂水平治疗后好转时则明显增高(TC:P<0.05,TG:P<0.05,HDL-C:P<0.05,LDL-C:P<0.05);30例治疗后恶化的AHF患者其血脂水平更加降低(P均<0.001)。108例AHF患者治疗前后血浆BNP水平较60例正常对照组显著增高(P<0.001),治疗后好转的明显降低(P<0.01),恶化的更加增高(P<0.001)。结论:作为一种指示性标志,血脂水平的显著降低和血浆BNP的显著增高,不仅是诊断和治疗考核AHF也能用作预后并指导治疗。Objective In order to study clinical significance of determination of changes of blood lipid and plasma BNP levels in patients with acute heart failure（AHF） both before and after treatment.Methods Blood lipid（with biochemistry method）and plasma BNP（with Chemiluminescene immunoassay）were determined in 108 patients with AHF before and after treatment as well as 60 normal controls and done compared analysis.Results The serum levels of TC,TG,HDL-C and LDL-C in 108 patients with AHF were significantly decreased than those in 60 controls before treatment（P all0.01）.After treatment,the 62 patients with AHF were much better,the blood lipid levels were obviously increased（P0.05~0.05）,but in 30 invalid patients,the blood lipid levels were continue decreased（P0.001）.Plasma BNP levels before and after treatment in 108 patients with AHF were significantly higher than those in 60 controls（P0.001）.After treatment the patients with AHF were much better,the plasma BNP level was significantly decreased（P0.01）,but in worsened patients with AHF the plasma BNP level was significantly increased（P0.001）. Conclusion As an indicative signal,the blood lipid and plasma BNP levels can not only to evaluate patient with AHF,but also predict the prognosis and instruct the therapy.
