摘 要:目的通过观察外源性sHLA-G体外干预对滋养细胞侵袭功能的变化,确定sHLA-G治疗特发性复发性流产患者的有效性及可行性进行研究。方法取早孕人工流产绒毛滋养细胞进行体外培养,sHLA-G进行干预,分别以生理盐水及5%丙种球蛋白干预为对照,流式细胞仪检测CD146黏附分子浓度,观察sHLA-G对体外培养滋养细胞亲润功能的变化。结果分别以0.2μg/mL、0.5μg/mL、1μg/mL不同浓度干预绒毛外滋养细胞EVCT,CD146阳性率分别为18.6%、27.1%、62.3%;绒毛滋养细胞VCT检测CD146阳性率分别为6.1%、7.6%、39.1%。生理盐水干预的EVCT检测CD146阳性率为9.3%;VCT检测CD146阳性率为13.0%。5%丙种球蛋白分别以3 mg/mL、6 mg/mL不同浓度干预EVCT,检测CD146阳性率分别为56.9%、61.7%;VCT检测CD146阳性率分别为15.8%、30.9%。结论外源性sHLA-G主动免疫治疗特发性复发性流产患者胎盘绒毛滋养细胞具有效性和优越性,可作为丈夫淋巴细胞细胞主动免疫治疗无效及丙种球蛋白特异性抗体效价不高治疗无效时的2线治疗措施。【Objective】 To research the variation of the invasion function of trophoblastic cell intervened by the exogenous sHLA-G in vitro,we will evaluate the validity and the feasibility on the treatment of the soluble HLA-G active immunotherapy in IRM.【Methods】 Fist trimester placental villus-derived trophoblastic cell were cultivated,intervened by soluble HLA-G,normal saline,gamma globulin respectively in vitro.We investigated the invasion function variation of trophoblastic cell intervened by sHLA-G in vitro,by analyzing CD146 using flow cytometry.【Results】 The positive rate of CD146 in EVCT intervened by 0.2 μg/mL,0.5 μg/mL,1 μg/mL,sHLA-G respectively is 18.6%,27.1%,62.3% respectively,and the positive rate of CD146 in VCT is 6.1%,7.6%,39.1% respectively;positive rate of CD146 in EVCT intervened by normal saline is 9.3%,CD146 in VCT is 13.0%;the positive rate of CD146 in EVCT intervened by 3 mg/mL,6 mg/mL,gamma globulin respectively is 56.9%,61.7%,and CD146 in VCT is 15.8%,30.9% respectively.【Conclusions】 It is concluded that the increase of the serum HLA-G by sHLA-G injection results in formation of abnormal immune storm,and plays a role in the pathogenesis of IRM,taking the place of the invalid treatment of husband lymphocytes active immunotherapy and low level of specific antibody in the gamma globulin.
