加味定喘汤治疗慢性阻塞性肺疾病急性加重期疗效观察

摘　　要：目的:观察自拟加味定喘汤治疗慢性阻塞性肺疾病急性加重期(AECOPD)的临床效果。方法:将本院呼吸科收治的82例AECOPD患者作为研究对象,随机分成2组,对照组40例给予抗感染、平喘、化痰、吸氧等常规西医治疗;治疗组42例在常规吸氧、抗感染的基础上,给予加味定喘汤治疗,治疗10天后观察2组临床疗效以及呼吸困难评分、肺功能、动脉血气分析指标的变化。结果:临床疗效总有效率治疗组为95.2%,对照组为75.0%,2组疗效比较,差异有非常显著性意义(P<0.01)。2组治疗后呼吸困难评分、肺功能及动脉血气各指标分别比较,差异均有非常显著性意义(P<0.01),治疗组改善明显优于对照组。结论:加味定喘汤具有清肺平喘、止咳化痰的功效,配合常规西医疗法治疗AECOPD,可以有效地控制临床症状,改善肺功能,减少复发率,值得临床推广。Objective： To observe the effect of modified Dingchuan Decoction for acute exacerbation of chronic obstructive pulmonary disease（COPD）. Methods, Eighty-two qualified cases were divided into two groups. Forty patients in the control group received routine western medical treatment including controlling infection, relieving asthma, resolving phlegm, and oxygen inhalation. Forty-two patients in the treatment group were treated with modified Dingchuan Decoction on the basis of oxygen inhalation and controlling infection. Therapeutic effect, lung function and blood gas analysis indexes were observed after treatment for ten days. Results： The total effective rate was 95.2% in the treatment group, and was 75.0% in the control group, the difference being significant （P 〈 0.01）. The differences of dyspnea score, lung function and blood gas analysis indexes were significant between the two groups after treatment（P 〈 0.05）. Conclusion：Modified Dingchuan Decoction has the actions of clearing lung and relieving asthma, arresting cough and resolving phlegm. When used together with anti-infection western medicines Modified Dingchuan Decoction can effectively control the clinical symptoms,improve lung function, and reduce the recurrence rate, which is worth of clinical extension.
