摘 要:目的探讨医院体检人群颈围与中心性肥胖测量指标的关联性。方法对4236名体检者进行体格检查,以颈围的均值四分位数分组,对各组年龄、身高、体重、体质指数(BMI)、腰围、臀围、腰臀比等的均值平均变化量进行比较。以腰围、BMI、腰臀比三种肥胖标准进行分组,分析颈围与肥胖的关系。以ROC曲线分析法判断颈围的切点。结果男性颈围为(37.40±2.46)cm,女性为(32.46±2.24)cm,男性大于女性(t=47.20,P<0.01)。随着颈围均值每四分位数的增加,男性和女性年龄、身高、体重、BMI、腰围、臀围、腰臀比的平均变化量均呈增加趋势,差异均有统计学意义(P<0.05),其中腰围(男性:5.58cm,女性:5.26cm)、体重(男性:6.25kg,女性:4.81kg)的平均变化量最大。三种肥胖标准分组中肥胖组的颈围均大于正常组,差异均有统计学意义(P<0.01)。对于中心性肥胖,男性颈围的切点是36.8cm,女性颈围的切点是33.1cm。结论颈围与年龄、身高、体重、BMI、腰围、臀围、腰臀比呈正相关关系。颈围与中心性肥胖存在显著联系,男性颈围>36.8cm或女性颈围>33.1cm时可以预测中心性肥胖。Objective To investigate the relationship between neck circumference (NC) and central obesity. Methods A cross-sectional investigation was conducted. 4 236 subjects receiving physical health examination were divided into four subgroups according to their NC quartiles. The average variations of means of age, height, weight, body mass index (BMI), waist circumference (WC), hip circumferences (HC), and waist-to-hip ratio (WHR) were compared among the subgroups; the relationship of NC with obesity was analyzed between subgroups divided by three obesity standards according to WC, BMI and WHR. And the cut-off value of NC for obesity diagnosis was analyzed with ROC method. Results The NC of male subjects (37.40±2.46) em was larger than that of the female (32.46± 2.24) cm (t=47.20, P〈0.01). With the increase of NC quartiles, the age, height, weight, BMI, WC and HC in male and female also increased (P〈0.05), and the average variation of WC and weight were the largest. Among subgroups according to three obesity standards, the NC in the obesity subgroup was larger than that in the normal group (P〈0.01);For the people with central obesity, the cut-off value of NC was 36.8 cm for men and 33.1 cm for women. Conclusion NC is positively related to age, height, weight,BMI, WC and HC; NC is related to central obesity; NC〉36.8 cm in men and NC〉33. 1cm in women may predict the risk for central obesity.
