彩色多普勒超声检查对涎腺肿块的诊断价值

摘　　要：目的:探讨彩色多普勒超声检查对涎腺肿块的诊断价值。方法:收集彩超高频探头检查并经穿刺活检或手术病理证实的涎腺肿块66例,分析其声像图表现,与病理作对照,并和CT检查作对比。结果:二维超声对病灶的检出率为100.0%;良性判断符合率为81.3%,恶性判断符合率为72.2%,总符合率为78.8%;良性判断误诊率为18.7%,恶性判断误诊率为27.8%,总误诊率为21.2%;与CT检查比较无明显差异。结论:以二维声像图为基础,结合肿块质地和活动度等相关信息,参考彩色多普勒血流成像术及频谱多普勒测值,彩色多普勒超声检查对涎腺肿块具有重要价值,是首选的影像学检查方法。Objective：To discuss the value of color Doppler ultrasonography（CDUS） in the diagnosis of salivary gland masses.Methods：The ultrasonographic images in 66 cases of salivary gland masses,proved by biopsy,operation or pathologic examination,which were analyzed and compared with CT examination results.Results：The detection rate of in salivary gland masses was 100.0% by the CDUS.The diagnostic accuracy in the benign masses was 81.3%,and in the malignant masses was 72.2%,the total accuracy reached at 78.8%.The misdiagnose rate in the benign masses was 18.7%,and in the malignant masses was 27.8%,the total misdiagnose rate was 21.2%.No significant difference was found between CDUS and CT examination results.Conclusion：Combining the color Doppler flowing image（CDFI） and pulse of Doppler measurement results,founding on the two-dimensional ultrasound image,the CDUS has showed an important value in the diagnosis of salivary gland masses,it should be considered as the first method of imaging study in the salivary gland masses.
