摘  要:目的通过观测不同病程高海拔地区老年慢性阻塞性肺病(COPD)患者血清胱蛋白酶抑制剂C(CystatinC,CysC)及血、尿β2-微球蛋白(β2-MG)水平的变化,探讨它们对肾功能损害的早期诊断价值。方法采用颗粒散射免疫比浊法、放射免疫法测定处于不同病程高海拔地区老年COPD患者血清CysC及血、尿β2-MG水平,并与30例健康人组行对比分析。结果与对照组相比,高海拔地区老年COPD患者血清CysC及血、尿β2-MG水平在血气分析正常期、单纯低氧血症期、低氧伴二氧化碳潴留期呈渐增趋势,而常规肾功能指标则无显著性差异(P>0.05)。结论及时测定血清CysC及血、尿β2-MG水平是监测高原地区老年COPD患者早期肾功能损害的敏感指标。Objective To investigate the value of early diagnosis by means of serum Cystatin C and blood and urine levels and beta 2 - micro globulin detection in different course of geriatric chronic obstructive pulmonary disease (COPD)complicated with renal damage at plateau. Methods Using particle scattering immune turbidity method and radioimmunoassay to measure CysC, blood and urine beta 2 - MG in elderly patients with COPD at different courses and compared with them with 30 healthy people. Results Compared with the control group, Cystatin C levels and blood serum, urine beta 2 - MG level increased during the stage of normal blood gas, the pure hypoxemia period and low oxygen with carbon dioxide retention period in elderly patients with COPD. But there was no significant difference in conventional renal index ( P 〉 0.05 ). Conclusion Cystatin C , blood and urine beta 2 - micro globulin are sensitive indicators for diagnosing early renal damage in patients with COPD at plateau.
