早期机械通气对高血压脑出血术后的影响

摘　　要：目的观察早期机械通气对高血压脑出血术后呼吸功能障碍患者预后的影响。方法回顾性分析我院71例早期应用呼吸机对高血压脑出血术后的影响。结果 71例患者均行早期机械通气,56例呼吸功能恢复正常,15例死亡。抢救成功率78.9%。机械通气治疗后较治疗前PO2、PCO2、SaO2、HR、R及GCS、GOS评分等指数均有显著改变(P<0.05),颅内压(ICP)通气治疗后与治疗前比较经统计学处理无显著性意义(P>0.05)。结论机械通气能较早改善高血压脑出血术后患者的缺氧症状,能暂时降低颅内压,缓解高血压脑出血术后的继发性脑损害。Objective To observe early mechanical ventilation on postoperative hypertensive cerebral hemorrhage complicated with respiration disorder and prognosis of the disease. Methods A retrospective analysis in our hospital from 2005 to 2010 was made in 71 cases of early ventilator on the impact of postoperative hypertensive cerebral hemorrhage. Results 71 patients were early mechanical ventilation, 56 cases of respiratory function returned to normal, 15 patients died. Success rate was 78.9%. comparison of after and before mechanical ventilation showed that PO2, PCO2, SaO2, HR, R, GCS and GOS score and other indices were significantly changed （P 〈 0.05） after treatment. But the intracranial pressure （ICP） has not changed after ventilation （P 〈0.05）. Conclusion The early mechanical ventilation can improve hypoxia and temporarily reduce intracranial pressure of patients with postoperative hypertensive cerebral hemorrhage. And it may release the secondary damage for postoperative hy- pertensive cerebral hemorrhage.
