踝、趾臂指数与下肢动脉超声检查联合诊断2型糖尿病的价值

摘　　要：目的探讨踝肱指数(ABI)、趾臂指数(TBI)、彩色多普勒超声检查在2型糖尿病(T2DM)患者中的价值。方法 67例2型糖尿病(T2DM)患者进行ABI、TBI测定,双下肢动脉彩色多普勒超声检查,记录患者的高血压、血脂异常等病史,根据ABI、TBI、超声检查和临床资料分组,进行统计学分析。结果 ABI、TBI值越低,下肢血管超声显示病变程度越重;超声对下肢血管病变检出率远远高于ABI,P<0.01;T2DM中,ABI和TBI在年龄、病程、有无高血压、血脂异常病史比较差异有统计学意义。结论 ABI,TBI及血管超声检查均能较好地反映DM患者的下肢血管病变,彩色多普勒超声对T2DM患者下肢血管病变的早期诊断及预防有临床价值;T2DM患者的ABI、TBI受年龄、糖尿病病程、高血压、血脂异常等因素影响。Objective To study the changes of ankle-brachial index（ABI）,toe-brachial index（TBI） and peripheral arterial ultrasound in type 2 diabetes（T2DM）.Methods ABI and TBI were measured by personal vascular lab on 67 subjects with T2DM,peripheral arterial ultrasound were measured by using color pulsed Doppler ultrasound examination.In the diabetes duration,hypertension and dyslipidemia were recorded.67 subjects with T2DM were divided into varied groups according to the results.Results The ABI and TBI were lower,the peripheral arterial diseases were higher.The vessel lesion incidence was significantly higher in T2DM than that in control group by color Doppler ultrasound examination（P0.01）.The ABI was significant correlation with age and duration of diabetes;there were significant difference in hypertension and dyslipidemia between ABI and TBI.Conclusion The changes of ABI,TBI and ultrasound examination is contest in peripheral arterial diseases with T2DM,and CDFI is an effective method,and the diabetes duration,hypertension,dyslipidemia can affect ABI and TBI in patients with T2DM.
