老年弥漫性间质性肺病临床特点分析

摘　　要：目的探讨老年弥漫性间质性肺病(ILD)的发病及临床特点。方法选择2006年1月至2011年1月经胸部影像学及临床相关检查确定无明显病因的老年ILD患者145例,分析其发病及临床特点,并将特发性肺间质纤维化(IPF)与非IPF进行比较分析。结果 145例老年ILD患者有呼吸道症状者53.1%,其中有呼吸道感染者79.2%;无呼吸道症状者29.7%,体检发现17.2%。既往有肺气肿38.6%,慢性阻塞性肺疾病36.6%;胃食管反流病24.1%;有吸烟史64.8%。主要体征:Velcro啰音52.4%、杵状指22.1%、发绀13.1%;进行肺功能检查者92例,肺功能异常者83.7%。呼吸道症状、肺气肿、吸烟、主要体征及肺功能异常者IPF组高于非IPF组。结论老年ILD起病隐匿;吸烟、慢性肺部疾病、呼吸道感染以及胃食管反流病与其发病有一定关系;IPF组临床表现及肺功能异常者更明显。Objective To investigate prevalence and clinical features in elderly patients with diffuse interstitial lung diseases（ILD）.Methods The data of 145 patients with ILD were analysed in retrospective study.All the patients with no obvious cause of disease,which was confirmed by HRCT and clinical examination,were divided into IPF and other ILD groups.Results Among 145 patients ILD,53.1% were associated with respiratory symptoms,79.2% were caused by respiratory infection,29.7% were accompany with other symptoms,17.2% had no symptoms;past histories showed 38.6% with emphysema,36.6% with COPD,24.1% with gastroesophageal reflux and 64.8% with smoking.Ratio of main physical symptoms was as follows：Velcro rales 52.4%,finger clubbing 22.1%,cyanosis 13.1%,abnormal pulmonary function 83.7%,of 92 cases of finished pulmonary function test.Incidence rates of respiratory symptoms,emphysema,smoking history,main physical symptoms and abnormal pulmonary function in IPF group were higher than that of other ILD groups.Conclusions ILD in the elderly is occult in onset;smoking history,chronic pulmonary disease,respiratory infection and gastroesophageal reflux might be related to the development of ILD;patients with IPF more obviously show abnormal pulmonary function and clinical manifestation.
