利培酮药物浓度监测及正常范围参考值的初步建立

摘　　要：目的通过大样本的治疗药物浓度监测,建立利培酮的正常范围参考值,探索其部分相关因素。方法收集利培酮治疗2周以上的住院病例的高效液相色谱技术(HPLC)-电化学方法测定所得的利培酮和其代谢产物9-羟利培酮(9-OH-RSP)浓度数据,共3 670例,采用SPSS17.0软件分析。结果 18～60岁组女性9-OH-RSP浓度(P<0.01)及其与利培酮浓度的比值(9-OH-RSP/利培酮)高于同龄男性,差异有统计学意义(P<0.01);18～60岁组男性和女性9-OH-RSP(男性P<0.05,女性P<0.01)和利培酮(男性P<0.05,女性P<0.01)浓度均高于60岁以上同性别病例组;多因素方差分析结果显示,年龄、性别和联合用药是影响利培酮血药浓度的显著性因素。结论通常剂量下利培酮及其主要活性代谢物(9-OH-RSP)和两者比值的正常范围参考值的初步探索证实,利培酮血药浓度与年龄、性别和联合用药存在一定程度的相关,值得进一步探讨。Objective To establish the reference range of serum concentration by therapeutic drug monitoring of risperidone and 9-hydroxyrisperidone （9-OH-RSP） in a large quantity of patients, and to investigate some related factors. Methods A total of 3 670 patients treated by risperidone for at least two weeks were collected. The serum concentration of risperidone and 9-OH-RSP were determined by high-performance liquid chromatography with electrochemical detection. Datas were analyzed by SPSS17.0 software. Results 9-OH-RSP serum concentration （P〈0.01） and the ratio of 9-OH- RSP/risperidone （P〈0.01） in female aged 18 to 60 years were significantly higher than those in male at the same age; The serum concentration of 9-OH-RSP and risperidone in both female and male aged 18 to 60 years were significantly higher than those in aged over 60 years （female, P〈0.01; male, P〈0.05） ; Multivariate Tests showed that risperidone serum concentration was related to age, gender and drug combination. Conclusion The reference range in serum concentration of risperidone has been set up and the risperidone serum concentration was related to age, gender and combination drug therapy.
