摘 要:目的:观察盐酸司维拉姆对慢性肾衰竭大鼠钙磷代谢和胎球蛋白A水平的影响。方法:30只SD雄性大鼠分别行一步法5/6肾切除(n=20,手术组)或假手术(n=10,对照组)3,0只大鼠术后2周开始给予高磷饮食6周。动物分3组:模型组(手术n=10),治疗组(手术+司维拉姆,n=10),对照组(假手术n=10),司维拉姆组给予司维拉姆(0.2%)治疗。高磷饮食配方:磷(P)1.2%,钙(Ca)1.6%,维生素D31000U/kg。饮食干预开始和结束时检测Scr、血P、血Ca、钙磷乘积及胎球蛋白A。结果:术后第二周饮食干预开始时,手术组大鼠Scr明显高于对照组大鼠(P<0.05),血P、血Ca、钙磷乘积及胎球蛋白A水平两组间差异无统计学意义。饮食干预六周时,血磷模型组最高,治疗组最低(P<0.05);各组间血钙水平差异无统计学意义;胎球蛋白A模型组明显低于治疗组(P<0.05)、对照组(P<0.05)。结论:盐酸司维拉姆能降低慢性肾衰大鼠高血磷及钙磷乘积,减轻钙化抑制物胎球蛋白A水平下降,可有效预防血管钙化的发生和进展。Objective: To investigate the effects of sevelamer hydrochloride on calcium-phosphorous metabolism and serum fetuin A concentration in chronic renal insufficiency rats. Method: 30 male SD rats either underwent 5/6 nephrectomy(n=20) or sham operation(n=10),and then divided into 3 groups: model group(5/6 nephrectomy,n=10),treatment group(5/6 nephrectomy with sevelamer treatment,n=10) and sham group(sham operation,n=10).All rats were fed with hyperphosphoruos diet 2 weeks after the operation.Serum creatinine,calcium,phosphorus,Fetuin A concentration and calcium-phosphorous product were checked before and after experiment. Result:Serum creatinine concentration was significantly higher in 5/6 nephrectomy rats than that in sham rats(P〈0.05),calcium,phosphorus,Fetuin A concentration and calcium-phosphorous product were no different before hyperphosphoruos diet.6 weeks after hyperphosphoruos diet,Serum phosphorus concentration was the highest in model group and lowest in treatment group(P〈0.05),Fetuin A concentration was lower in model group than that in treatment group(P〈0.05)and sham group(P〈0.05).Calcium concentrations were no different among 3 groups. Conclusion: Sevelamer hydrochloride decreased serum phosphorus concentration and calcium-phosphorous product,lightened the decrease of fetuin A concentration in chronic renal insufficiency rats and may prevent vascular calcification.
