先天性漏斗胸患者胸廓畸形程度与肺功能变化的研究

摘　　要：目的探讨先天性漏斗胸患者肺功能变化与胸廓畸形程度的关系。方法纳入四川大学华西医院2008年1月至2010年7月期间65例患者胸部CT及肺功能检查资料,进行回顾性分析。结果先天性漏斗胸患者的肺功能各指标除PEF、FEF50、FEF75、BR轻度下降,RV、RV/TLC、Raw增高外,其余指标均在正常范围内。比较不同畸形程度的患者发现FEF75随胸廓畸形程度越重降低越多且超出正常范围,虽然FEV1、FEF25-75、VC也会因胸廓畸形程度越重降低越多,但仍在正常范围内。结论先天性漏斗胸轻、中度患者肺功能检测多为正常,重度患者有轻度下降。Objective To investigate the relationship betwee n the severit y of deformities and lung function in patients with congenital pectus excavatum. Methods The spiral CT and respiratory function data of 65 su rgically correcte d pectus excavatum patients during January 2008 and July 2010 were collected and retrospectively reviewed.Results Peak expiratory flow（PEF）,forced expirat ory flow at 50%（FEF50）,FEF75,and breathing reserve ratio（BR） were decreased while residual volume（RV）,residual volume to total lung capacity ratio（RV/T LC）,and airway resistance（Raw） were increased.The FEF75 increased with th e seve rity of deformities.Forced expiratory volume in 1 second（FEV1）,forced exp ir atory flow at 25% to 75% of forced vital capacity（FEF25-75）,and vital capaci ty（VC） increased with the severity of deformities too,but still in the normal range.Conclusion The lung function is relativ ely normal in the patients with mild to moderate congenital pectus excavatum,and only decreased mildly in severe cases.
