摘 要:目的旨在评价不同管电流量的64排CT鼻窦图像质量及低剂量CT的临床价值。方法选取临床怀疑副鼻窦病变而行64排CT检查的122例患者,其中男性68例,女性54例;年龄15~70岁,平均年龄35.5岁。随机分为4组。扫描设备为东芝Aquillion64排螺旋CT机。其他扫描条件固定,每组分别以110、20、15、10mAs管电流量进行扫描。双盲法测量各组重建图像的噪声、评价图像质量及各解剖结构的显示情况,并进行统计学分析,P<0.05为差异有统计学意义。结果随着管电流量的降低,各组总毫安量和辐射剂量长度(DLP)相应减少,图像噪声增加,各组间差异具有统计学意义(P<0.05),而各组间图像评价和各解剖结构评分差异却无明显统计学意义(P>0.05)。结论副鼻窦非肿瘤性病例采用64排CT低剂量扫描后图像噪声增加,但是图像质量和各解剖结构的显示能够满足诊断要求,而患者所受辐射剂量有明显的减少,有重要临床价值。Objective To evaluate the image quality of paranasal sinuses by low-dose scan of 64-detector row computer tomography and its clinical value.Methods A total of 122 patients with paranasal sinus diseases were enrolled,male 68,female 54;aged 15-70 years old,mean age 35.5 years old,which examined by Toshiba Aquillion 64-detector row CT.All of them collected and divided into 4 groups(110 mAs,20 mAs,15 mAs and 10 mAs) randomly.Other scan parameters were fixed.The reconstruct image noises and quality as well as the visualization of anatomical landmark of paranasal sinus were evaluated by 2 advanced radiologists with double-blind method.All the results were analyzed with SPSS 11.5 package.Results Followed the reduction of tube currents,the total mAs and total dose length product(DLP) were decreased while the image noise increased.There were significant statistical differences in total mAs,DLP,and image noise between each of 2 groups(P 〈 0.05).Moreover,there were no statistical difference of image score and anatomical landmark between each of 2 groups(P 〉 0.05).Conclusion It is demonstrated that the low-current protocol resulted in a significant reduction in radiation exposure at paranasal sinuses CT.The image quality and anatomy display are satisfied with the diagnostic requirement,and radiation dose of patient is significantly reduced,and which has important clinical value.
