健康教育对社区高血压患者疗效影响的Meta分析

摘　　要：目的综合定量评价健康教育对社区高血压患者疗效的影响。方法通过计算机检索中国生物医学文献数据库(CBM)、中国期刊全文数据库(CNKI)、万方数据库、维普中文科技期刊全文数据库(VIP)1999-2010年的文献资料。按照纳入标准的要求筛选相关文献,选择健康教育干预后健康教育组和对照组社区高血压患者的收缩压和舒张压的均值和标准差作为评价指标,使用Rev Man5.0统计软件进行Meta分析。结果共有7篇文献符合入选标准,采用随机效应模型,加权均数差(WMD)进行Meta分析。结果显示健康教育组与对照组在收缩压[WMD=-18.27,95%CI(-25.30,-11.24),P<0.00001]和舒张压[WMD=-8.77,95%CI(-12.00,-5.55),P<0.00001]方面差异有统计学意义,敏感性分析显示本评价结果较稳定。结论健康教育是促进社区高血压患者健康的重要措施,在常规药物治疗的基础上可以使血压控制在更有利的水平,由于纳入分析的研究质量较低,需要更多高质量的随机对照试验进一步论证。Objective To comprehensively evaluate the curative effect of health education for hypertension patients in communities by using the Meta-analysis. Methods The CBM,CNKI,Wanfang,and VIP databases were searched,and related articles published between 1999 and 2010 were selected according to inclusion criteria.The mean value and standard deviation of systolic and diastolic blood pressure of hypertension patients in communities between health education group and the control group after health-educational intervention were chosen as indicators.The Rev Man5.0 software was taken for Meta-analysis. Results A total of seven articles met inclusion criteria.Random effects model and weighted mean differences（WMD） were chosen for meta-analysis.The Meta-analysis showed that there were statistically significant differences between the health education group and the control group in systolic blood pressure [WMD=-18.27,95%CI（-25.30,-11.24）,P〈0.00001],and diastolic blood pressure [WMD =-8.77,95% CI（-12.00,-5.55）,P〈0.00001].Sensitivity analysis showed that the results of the evaluation were stable. Conclusions Health education is an important measure to promote the health of hypertension patients in communities,and it may control blood pressure in a more favorable level on the basis of routine therapy.However,most trials included in the review are of low quality,so large-scale randomized controlled higher-quality trials are needed to confirm the results.
