摘 要:目的:分析瞬态诱发性耳声发射(transient evokedotoacoustic,TEOAE)联合听性脑干诱发电位(automated au-ditory brainstem response,AABR)在婴儿听力筛查中的作用。方法:对TEOAE两次听力筛查未过的1 560例(3 120耳)婴儿,运用TEOAE联合AABR分别在婴儿3个月、5个月时进行听力筛查。结果:在3 120耳(1 560例)中,3个月大时2 652耳通过TEOAE和AABR联合筛查,进入随访阶段;余468耳在5个月大时46耳通过联合筛查,进入随访阶段;422耳转上级医院进一步诊断及治疗。在422耳中,398耳未通过联合筛查,其中149耳诊断为分泌性中耳炎,18耳诊断为大前庭水管综合征,231耳诊断为感音神经性耳聋;TEOAE通过而AABR未通过的2耳,诊断为小儿听神经病;TEOAE未通过而AABR通过的22耳中,11耳诊断为分泌性中耳炎,11耳听力正常。结论:TEOAE和AABR联合应用可以优势互补,减少误诊、漏筛率,有利于听力障碍儿的早期发现与诊断,是婴幼儿听力筛查的重要方法之一。Objective:To analyze the effect of transient evoked otoacoustic emission(TEOAE) combined with automated auditory brain stem response(AABR) in infantile hearing screening.Methods:1 560 infants(3 120 ears) who failed in hearing screening with TEOAE twice received hearing screening at 3 and 5 months after birth with TEOAE combined with AABR.Results:Among 1 560 infants(3 120 ears),2 652 ears passed hearing screening of TEOAE combined with AABR at 3 months after birth,then the infants entered into follow-up stage;46 ears passed hearing screening of TEOAE combined with AABR at 5 months after birth,then the infants entered into follow-up stage;422 ears received further diagnosis and treatment in higher level hospitals.Among 422 ears,398 ears didn't pass combined hearing screening,including 149 ears diagnosed as secretory otitis media,18 ears diagnosed as large vestibular aqueduct syndrome,231 ears diagnosed as sensorineural hearing loss;2 ears passed TEOAE and then failed in AABR,they were diagnosed as auditory neuropathy;22 ears passed AABR and then failed in TEOAE,including 11 ears diagnosed as secretory otitis media and 11 ears with normal hearing.Conclusion:TEOAE combined with AABR can complement each other's advantages,reduce misdiagnosis rate and missed diagnosis rate,and it's helpful to early detection and diagnosis of infants with hearing disorder,the method is important for infantile hearing screening.
