IGFBP-1对两种浓度缩宫素在延期妊娠促宫颈成熟引产效果的检测

摘　　要：目的:探讨延期妊娠孕妇静脉滴注不同浓度缩宫素促宫颈成熟引产效果。方法:选择120例孕41～41+6周延期妊娠第一胎孕妇随机分成两组,各用1种浓度缩宫素引产,观察宫颈Bishop评分、宫颈分泌物IGFBP-1的变化,比较其引产效果。结果:两种浓度缩宫素促宫颈成熟率、IGFBP-1测试阳性率、分娩率差异无统计学意义(P>0.05),对宫颈Bishop评分≥7分者缩宫素促宫颈成熟率、IGFBP-1测试阳性率及其阳性分娩率均高于同组≤6分者,差异有统计学意义(P<0.05),其分娩率与IGFBP-1测试阳性结果呈一致性。结论:低浓度缩宫素对延期妊娠孕妇促宫颈成熟引产效果可靠,宫颈分泌物IGFBP-1测试阳性是宫颈成熟、分娩即将发动的标志。宫颈Bishop评分≤6分孕妇宜选用0.2%缩宫素促宫颈成熟,宫颈Bishop评分≥7分、IGFBP-1测试阳性孕妇宜选用0.5%缩宫素促宫颈成熟引产。Objective：To explore the cervical ripening promotion and induced abortion effects of oxytocin of different concentrations for women of prolonged pregnancy.Methods：120 primiparous women of 41~41＋6 gestational weeks with prolonged pregnancy were divided into two groups randomly,the women in the two groups were treated with oxytocin of two different concentrations,cervical Bishop score and the change of insulin-like growth factor binding protein-1（IGFBP-1） in cervical secretions were observed,the induced abortion effects were compared between the two groups.Results：There was no significant difference in the rate of cervical ripening,positive rate of IGFBP-1 test and delivery rate between the two groups（P0.05）,the rate of cervical ripening,positive rate of IGFBP-1 test and positive delivery rate in women with cervical Bishop score≥7 were significantly higher than those in women with cervical Bishop score≤6（P0.05）,the delivery rate was in accordance with positive result of IGFBP-1 test.Conclusion：The cervical ripening promotion and induced abortion effect of oxytocin of low concentration is reliable,positive IGFBP-1 test in cervical secretions is a marker of cervical repining and onset of delivery.The pregnant women with cervical Bishop score≤6 are better to choose 0.2% oxytocin,while the pregnant women with cervical Bishop score≥7 and positive IGFBP-1 test are better to choose 0.5% oxytocin.
