电动充气止血带释放时机对膝关节交叉韧带重建术后患肢功能恢复的影响

摘　　要：[目的]探讨电动充气止血带不同释放时机对交叉韧带损伤患者术后患肢功能恢复的影响。[方法]实验选取临床上膝关节前、后交叉韧带损伤的患者56例(前交叉韧带损伤患者47例,后交叉韧带损伤患者9例),平均年龄(26.75±5.84)岁。随机分成A、B两组。A组样本量为33例,术中止血带释放先于手术创口关闭;B组样本量为23例,术中止血带于手术创口关闭后释放。术后2～3个月对患者进行患肢功能恢复的评估,采用Marshall评分系统,对患者术后患肢功能恢复情况评分,设定评分系统中>35分为术后效果满意。统计学数据分析采用t检验以及χ2检验。P<0.05,则两组间比较差异有统计学意义。[结果]根据评定结果显示,A组(n=33)患者术后评价平均得分为(36.79±6.23)分;B组(n=23)患者术后评价平均得分为(32.75±7.02)分。根据t检验分析,A、B两组得分差异显著,A组平均得分高于B组。A组评分>35分的患者为24人,占该组总人数的72.7%;B组评分>35分的为10人,占该组总人数的43.5%。经统计学分析,两组间差异有统计学意义。A组满意率高于B组。P<0.05。[结论]不同止血带释放时机对患者术后患肢短期恢复情况有影响。手术创口关闭前释放止血带较手术创口关闭后再释放止血带对患者术后患肢恢复更为有利。[Objective] To investigate the influence of the timing of pneumatic tourniquet release on functional recovery after the arthroscopic ligament reconstruction.[Method]We selected 56 patients with cruciate ligament injuries（47 with anterior cruciate ligament injuries and 9 with posterior cruciate ligament injuries） with the average age of 26.75±5.84 years and randomly divided them into two groups.Group A（n=33）,the tourniquet deflated intra-operatively prior to the wound closure and Group B（n=23）,the tourniquet deflated after the wound had been closed.The assessment of recovery was done 2-3 months after surgery,using Marshall Scoring Scale.In this study,the score above 35 points was regarded as satisfaction.The data were analyzed by t test and x2 test and all results were regarded as significant when P0.05.[Result]The mean score obtained by Group A（n=33） was 36.79±6.23 whereas the mean score of Group B（n=23） was 32.75±7.02.The difference between the two groups was regarded as significant according to the t test.The score of Group A was higher than that of Group B.The number of patients with the score above 35 in group A was 24,which was significantly more than Group B.The difference of satisfaction rate between the two groups was statistically significant（P0.05）.[Conclusion]Different timings of tourniquet release imposed influences on patients＇ short-term functional recovery for patients after arthroscopic cruciate ligament reconstruction.The release of tourniquet before the wound closure is more favorable to the functional recovery for patients than releasing after wound closure.
