37例腹腔镜膀胱全切原位回肠代膀胱患者自主排尿功能的训练

摘　　要：对37例腹腔镜膀胱全切原位回肠代膀胱患者术后行自主排尿功能训练。19例术后3个月内代膀胱控尿功能形成,日夜间均能完全控尿,每次排尿250ml以上;8例3个月时夜间熟睡后有轻度尿失禁,强化夜间定时闹钟训练1个月后好转;7例术后6个月达到完全控尿;2例术后10个月达到完全控尿;1例高龄患者术后1年代膀胱控尿功能仍未完全形成,出现清醒期尿失禁。认为代膀胱排尿功能训练是原位回肠代膀胱术后患者掌握和适应新排尿方式的有效途径,可以明显提高控尿和排尿能力,提高患者的生活质量。Bladder training was practiced in 37 patients with laparoscopic radical cystectomy plus ileal orthotopic neobladder. Diurnal and nocturnal voluntary micturitions were completely controlled in 19 patients in 3 months;8 cases had mild urinary incontinence at night sleeping and were improved after setting the alarm clock at night for one month;7 cases could complete control of urination after 6-month training;2 cases could complete control of urine after 10-month training;an aged patient still had a waking incontinence after 1-year training. Bladder training is proved to be an effective method to adapt to the new way of voiding in patients with laparoscopic radical cystectomy plus ileal orthotopic neobladder,which can increase the ability to control urine ,and improve patients＇ quality of life.
