血压正常高值人群代谢异常聚集对动脉硬度的影响

摘　　要：目的通过研究正常高值血压和代谢异常因素及其聚集对肱踝脉搏波速度(baPW V)的影响,有效地评价血压正常高值者动脉硬度的变化,探讨合并代谢异常及其聚集对动脉硬度的影响。方法入选血压正常高值人群和血压正常人群各1500例,比较两组baPW V及代谢异常发生率的差异。对血压正常高值人群中合并代谢异常的类型及代谢异常因素的累积情况进一步分亚组进行统计,比较各亚组baPW V的差异。结果与血压正常组比较,血压正常高值组的baPW V显著加快([1369±161.4)cm/s vs(1262±141.7)cm/s,P<0.001],其合并代谢异常的比例也明显升高(63.8%vs 51.1%,P<0.001)。对血压正常高值人群的代谢异常亚组进行比较,结果显示合并血脂异常、血糖异常或肥胖时,baPW V均显著高于无代谢异常的对照组(P<0.01),且baPW V随着代谢因素的增多而逐步增高。结论血压正常高值人群的动脉硬度已经开始增加;血压正常高值人群比血压正常人群合并代谢异常更常见,各种代谢异常均可促进动脉硬化的发生发展;合并代谢异常因素越多,对动脉硬度的负面影响越大。【Objective】 To investigate the association of metabolic risk factors with the brachial-ankle pulse wave velocity（baPWV） in prehypertensive people.【Methods】 We examined 1500 prehypertensive subjects and 1500 normotensive controls from the Health Center of The Third Xiangya Hospital.baPWV was measured by VP-1000.The triglyceride（TG）,total cholesterol（TC）,high-density lipoprotein-cholesterol（HDL-C）,low-density lipoprotein-cholesterol（LDL-C） and fasting blood sugar（FBS） were evaluated as well as blood pressure.【Results】 When compared with those in the normotensive group,the baPWV value and the incidence of metabolic risk factors in the prehypertensive group were significantly increased [（1369±161.4） cm/s vs（1262±141.7） cm/s and 63.8% vs 51.1%,respectively,P〈0.001].In the prehypertensive group,the risk factor of hyperlipidemia,hyperglycemia or obesity further increased baPWV when compared with those without metabolic disorders（P〈0.01）.Moreover,the more metabolic risk factors coexisted,the higher baPWV was found.【Conclusion】 Prehypertensive subjects have higher risks of metabolic disturbance with increased artery stiffness than normotensive subjects.The metabolic risk factors,such as hyperglycemia,obesity and hyperlipidemia,can raise baPWV.More metabolic risk factors further boost artery stiffness.
