非TIA非梗死性后循环缺血患者血流动力学变化及盐酸法舒地尔对其的临床疗效

摘　　要：目的:探讨非短暂性脑缺血发作(TIA)非梗死性后循环缺血患者血流动力学变化及盐酸法舒地尔对其的临床疗效。方法:将非TIA非梗死性后循环缺血患者180例,随机分为法舒地尔组与对照组,分别给予盐酸法舒地尔注射液和丁咯地尔注射液,疗程14d;应用彩色多普勒超声仪观察治疗前后双侧椎动脉、基底动脉平均血流速度变化,同时对比2组临床疗效。结果:治疗14d后,法舒地尔组总有效率83.33%明显高于对照组的67.78%(P<0.05);与治疗前比较,2组基底动脉、左侧椎动脉、右侧椎动脉平均血流速度均明显增加(P<0.01或P<0.05),法舒地尔组较对照组相应动脉平均血流速度增加明显(P<0.05)。结论:非TIA非梗死性后循环缺血患者椎-基底动脉系统平均血流速度降低,颅内微血管痉挛所致阻力增高可能是其病因之一;盐酸法舒地尔能缓解部分后循环缺血患者的临床表现,疗效确切,安全、有效。OBJECTIVE： To investigate clinical efficacy and changes of hemodynamics of Fasudil hydrochloride in patients with non-TIA and non-infarction posterior circulation ischemia （PCI）. METHODS： 180 patients with non-TIA and non-infarction PCI were randomly divided into fasudil group and control group. Two groups were given Fasudil hydrochloride injection and Buflomedil injection for 14 days. Mean blood flow velocity of basilar artery and bilateral vertebral artery were observed using color Doppler. And the clinical efficacies of two groups were compared. RESULTS： After 14 days of treatment, the total effective rate of fasudil group was significantly higher than that of control group （83.33% vs. 67.78%, P〈0.05）. The mean blood flow velocity of basilar artery and bilateral vertebral artery were significantly increased in two groups （P〈0.01 or P〈0.05）, and the mean blood flow velocity of fasudil group were markedly increased as compared with control group （P〈0.05）. CONCLUSION： The mean blood flow velocity of vertebrobasilar system is decreased in some patients with non-TIA and non-infarction PCI. The microvascular vasospasm may be one of causes of disease. Fasudil hydrochloride is effective and safe in relieving clinical symptom of some patients with non-TIA and non-infarction PCI.
