摘 要:目的了解马鞍山市金黄色葡萄球菌的分子特征和耐药性,为追踪传染源和临床用药提供依据。方法收集马鞍山市食物中毒来源、食品和腹泻患者等样本分离的金黄色葡萄球菌,用mini-VIDAS全自动荧光免疫分析仪检测菌株肠毒素,采用脉冲场凝胶电泳(PFGE)技术对菌株做分子分型,采用琼脂稀释法对菌株做药敏试验。结果本研究共收集到59株金黄色葡萄球菌中,肠毒素阳性有40株(67.8%);PFGE分型结果,除4株不能分型外,55株菌株分为33个型别,两起食物中毒的11株菌分别属于同一型别;44散发菌株产生33个带型,表现为多样性。但某些分离自不同时间的菌株可有相同的带型;或者在同一时间、同一地点分离的菌株,其带型可表现不同。12种抗生素药敏试验结果表明,菌株对去甲万古霉素100%敏感,对其余11种则不同程度的耐药,其中对青霉素、氨苄西林的耐药性较高。结论应加强对食品和腹泻患者中金黄色葡萄球菌的监测分析,以预防食物中毒的发生和追踪传染来源。Objective To understand molecular features and drug resistance of food source Staphylococcus aureus(S.aureus) and provide evidence for tracing the source of infection and clinical antibiotic application.Methods The strains of S.aureus were isolated from food and patients with food poisoning in Maanshan,Anhui.Enterotoxin was detected with miniVIDAS automated fluoroimmunoassay.The method pulsed-field gel electrophoresis(PFGE) was used for gene typing.The susceptibility of antibodies was tested for all isolated strains.Results A total of 59 S.aureus strains were isolated from food source and the patients of food poisoning.Enterotoxin was detected in 39(67.8%) strains.The method of PFGE successfully classified 55 isolates into 33 gene types with 4 isolates indeterminable.The diversity of strains from sporadic cases was confirmed except for 11 strains,which were linked to two outbreaks of food poisoning and belonged to the same gene type.Drug susceptibility was tested with 12 antibiotics.All the isolated strains were 100% sensitive to vancomycin and had higher resistance to penicillin and ampicillin.Conclusion A wide variety of S.aureus genomic types of toxigenic S.aureus could contaminate food and lead to food poisoning.The surveillance and ongoing monitoring of S.aureus should be strengthened to ensure food safety.
