后路侧块螺钉治疗寰枢关节脱位7例

摘　　要：目的探讨后路侧块螺钉治疗寰枢关节脱位的疗效和体会。方法采用后路寰椎侧块枢椎椎弓根螺钉内固定治疗寰枢关节脱位7例,其中单纯寰枢关节脱位2例,伴齿状突骨折的寰枢关节脱位5例,术后观察其力学稳定性和植骨融合情况。结果随访时间6个月～1年,平均9.8个月;所有病例植骨均愈合,未发生内固定移动、断裂和神经损伤;脊髓神经功能均有不同程度的恢复。结论采用后路寰椎侧块螺钉固定具有较好的力学稳定性,融合率高,是目前治疗寰枢关节脱位较理想的手术方法。[Objective] To investigate the curative effect and comprehension of inner fixation with lateral mass screws in atlantoaxial dislocation. [Methods] Internal fixation with lateral mass screws was applied to aflantoaxial dislocation in 7 cases, among whom two were with odontoid fracture and five with atlantoaxial dislocation. Stability and bonefusion status were investigated after operation. [Results] The follow-up duration was from 6 months to 1 year, with the average of 9.8 months. All the bones fused well. There were no signs of rupture, loosing of falling in the lateral mass screw, and the function of the spinal cord was recovered. [ Conclusion] The method of using lateral mass screw offers a strong inner fixation for treatment of atlantoaxial dislocation, which accelerates the bone fusion and is a better method for treatment of atlantoaxial dislocation.
