股骨近端锁定板治疗老年股骨转子间骨折

摘　　要：目的观察股骨近端锁定板治疗老年股骨转子间骨折的疗效。方法采用股骨近端锁定板固定治疗老年股骨转子间骨折29例。观察切口愈合情况、骨折复位和愈合结果、并发症情况、髋关节有无疼痛、行走功能恢复情况。结果所有病例均获得随访,随访时间为12个月～36个月,平均17个月。切口均一期愈合。骨折均愈合,愈合时间12周～16周,平均13.5周。X线片示骨折解剖复位23例,近解剖复位6例。无内固定松动及断裂、钉折弯。无下肢深静脉血栓形成病例。所有病例髋关节无疼痛,外观无明显畸形,髋关节活动度均达正常值80%以上,行走功能恢复率100%(26/26),患者及家属对步态和步速满意。结论股骨近端锁定板是治疗老年股骨转子间骨折较为理想的方法,其适应征广,操作方便。【Objective】 To observe the effect of proximal femoral locking plate fixation in the treatment of femoral intertrochanteric fracture in geriatic patients.【Methods】 Twenty-nine cases of femoral intertrochanteric fracture in geriatic patients were treated with proximal femoral locking plate.Then the reduction and bone union,the incision healing and walking function recovery was observed.【Results】 All cases were followed up from 12 to 36 months with the mean of 17 months.Postoperative X-ray showed that fracture anatomical reduction in 23 cases,function reduction in 6 cases.Incisions were primary healing.Bone unions were showed by the X-ray.The bone union time were from 12 to 16 weeks with the mean of 13.5 weeks.None had complications of loss of reduction,plate break,lower limbs of deep vein thrombosis and wound infection.None had hip pain.All cases who could walk pre-operation recovered the walking function.【Conclusion】 Proximal femoral locking plate fixation is a good choice to treat femoral intertrochanteric fracture in geriatic patients.
