改良三瓣法额肌瓣悬吊术治疗重度上睑下垂临床观察

摘　　要：目的探讨改良三瓣法额肌瓣悬吊术矫正重度上睑下垂的临床效果,恢复上睑功能和视功能,同时达到美容的目的。方法对36例(47只眼)重度上睑下垂患者行改良三瓣法额肌瓣悬吊的手术治疗。结果 36例(47眼)中,满意38眼,占80.85%,良好8眼,占17.02%,失败1眼,占2.13%。半年后对失败的上睑下垂患者予以2次手术,上睑下垂得到矫正。术后获随访6个月,均获得满意的效果,无并发症。结论改良三瓣法额肌瓣悬吊术治疗重度上睑下垂,力度分散,并发症少,效果良好,重睑自然,恢复快。Objective To determine the clinical effects of our modified three muscular frontal flap suspension surgery in the treatment of severe blepharoptosis in order to gain the function of upper eyelids and visual function,and cosmetic objective at the same time.Methods We modified the conventional surgery by cutting muscular frontal flap into 3 parts,and then suspending tarsal plate with the 3 parts in a fan shape.A total of 47 eyes of severe blepharoptosis（a muscular frontal flap of 1.5 cm,36 patients） received our modified three muscular frontal flap suspension surgery.Results Of 47eyes,46 eyes were cured after operation,occupy 97.87%.In the follow-up survey of 6 months,eyelids had restored to normal positions without any complication.Conclusion Modified three muscular frontal flap suspension operation is an ideal operation for severe blepharoptosis.
