在丙泊酚复合芬太尼麻醉下行无痛胃镜检查1564例

摘　　要：目的:探讨无痛胃镜检查应用的效果。方法:术前准备同常规胃镜检查,检查前仍为受检者咽部喷洒2%利多卡因三次,检查时先给病人静推2%利多卡因0.5mg/Kg、芬太尼0.5μg/Kg、丙泊酚(1.0～2.0)mg/Kg,待病人睫毛反应消失并呼吸血压平稳时即行胃镜检查。结果:本组1564例除少数病人出现进镜时躁动、呛咳外,其他均顺利完成胃镜检查,病人清醒后对受检过程均无知觉。检查完成时间(3～15)分钟,平均7.2分钟。结论:镇静催眠麻醉状态下行胃镜检查术可减少病人的紧张、焦虑和恐惧心理,减轻病人受检时的痛苦,降低高血压、冠心病及年老患者作胃镜时发生中风、心梗等危险,还能使儿童及精神病患者顺利完成检查,所以,无痛胃镜检查是一种安全、可靠、舒适、有效的方法。Objective： To explore the effects of painless gastroscopy examination.Methods： Preoperative preparation with conventional endoscopy, the subjects before the examination was still sprayed three times with 2% lidocaine throat, checking the patient gived bolus of 2% lidoeaine 0.5mg/kg, fentanyl 0.Sug/kg, propolbl （1.0- 2.01 mg/kg, eyelash loss of response to be patient and stable blood pressure when breaihing was endoscopy, Results： 1 564 cases in addition to a few patients into the mirror when the agitation, cough, the other were the successful completion of endoscopy, the patient had no conscious perception of the subjects of the process. Checked the completion time （3 to 15） minutes.an average of 7.2 minutes,Conclusion： Sedative hypnotic anesthesia down gastroscopy reduced the palient＇s stress, anxiety and fear. when subjects palicnts to reduce pain, reduce high blood pressttre, coronary heart disease and elderly patients for endoscopy when stroke, myocardial inthrction, etc, dangerous, but also children and the mentally ill to the successful completion of inspection, so painless endoscopy was a safe, reliable, comfortable, effective way.
