综合医院住院患者直接护理时间的测量及其影响因素的分析

摘　　要：目的:测量综合医院住院患者在优质护理服务规范要求下的直接护理时间,分析直接护理时间的影响因素,为护理工作量的测算和配置人力提供依据。方法:运用ADL量表和APACHEⅡ对患者进行评估分类,然后根据分类情况抽取患者进行直接护理时间测量,收集资料并进行统计分析。结果:该院住院患者直接护理时间平均需求为(125.58±43.44)分钟;不同ADL等级、护理等级和疾病严重程度病人直接护理时间差异有统计学意义(P<0.05);直接护理时间与ADL等级、疾病严重程度呈正相关,与护理等级呈负相关(P<0.05)。结论:该院患者在优质护理服务规范要求下的直接护理时间为(125.58±43.44)分钟,直按时间受患者疾病严重程度、护理等级、自理能力因素的影响,护理管理者在配置护理人力时,一定要了解患者直接护理时间需求,正确处理时间与影响因素之间的关系,合理配置护理人力。Objective： To measure the direct nursing time and analyze the effect factors of inpatiems at the behest of quality nursing service in general hospital, and to provide the evidcncc to nursing workload measurement and staffing.Methods：TheActivities of Daily Living （ADL）scale and acute physiology and chronic health evaination II （ APACHE II ）are used to evaluate inpatients and classify inpatients to several categories. Then to extract patients to measure direct nursing time bascd on the categories. All data were collected and statistically treated.Results：The average direct nursing time of patients was （125.58±43.44）minutes. The direct nursing time of patients of different ADL grade, different nursing degree and discase severity displayed obvious difference（P〈 0.05）. The direct nursing time was positively correlated with the ADL grade and disease sevcrily, and ncgatively correlated with the nursing degrce（P〈0.05）.Conelusion：The need of direct nursing time of inpatients at the behest of quality nursing service was （ 125.58 ± 43.44）minutes. Direct nursing time was influenced by the disease scvcrity, the nursing degree and the activities of daily living. Nursing managers should understand the patients＇ need of direct nursing time, deal with the correlation between nursing time and influence factors correctly, and arrange reasonable nurse when staffing.
