小儿纤维支气管镜对全肺灌洗术安全性的影响

摘  要:目的 观察小儿纤维支气管镜对提高尘肺大容量全肺灌洗(whole—lunglavage,WLL)安全性的应用价值。方法2006年5月至2010年5月应用小儿纤维支气管镜辅助插管定位的患者为纤维支气管镜组(104例次),1998年5月至2004年11月应用传统的听诊法导管定位的患者为对照组(376例次),比较两组在WLI.巾的不良反应及并发症。结果纤维支气管镜组发生漏液1例(0.96%),低氧血症3例(2.88%)和肺内残余液〉1000ml15例(14.42%),对照组发生漏液24例(6.38%),低氧血症42例(11-17%)和肺内残余液〉1000ml135例(35.90%),两组比较,差异均有统计学意义(P〈O.05,P〈0.01)。结论WLL中应用小儿纤维支气管镜探查,能保证双腔管对位准确,两肺分隔完全,明显减少并发症,提高WLL的安全性。Objective To explore the effect of children-sized fibreoptic bronchoscope in improving the safety of whole-lung lavage (WLL). Method Patients from May 2006 to May 2010 using children-sized fibre- optic bronehoscope to assistant the location were assigned to fibreoptie bronchoscope group. Patients from May 1998 to Nov 2004 using traditional stethoseopy to help intubation were assigned to control group. The adverse reactions and complications were compared. Result There were liquid leakage 1 case (0.96%), hypoxia 3 cases (2.88%) and liquid retained over 1000 ml 15 cases (14.42%) in fibreoptie bronchoscope group. In con- trast, liquid leakage 24 cases (6.38%), hypoxia 42 eases (11.17%) and liquid retained over 1000 ml 135 cases (35.90%) happened in control group. The differences between the two groups were significant (P〈0.05, P〈0.01 ). Conclusion Using children-sized fibreoptic bronchoscope in WLL can promote the situation of double-lumen tube, help separation the two lungs, decrease complications and improving safety.
