美罗培南治疗107例肺部感染中痰病原学变化的评价

摘　　要：目的评价美罗培南治疗肺部感染中的痰病原学变化。方法回顾性观察本院呼吸内科2010年出院的使用美罗培南治疗肺部感染患者的痰病原学变化。结果美罗培南清除的痰病原包括铜绿假单胞菌、肺炎克雷伯氏菌、鲍曼不动杆菌、洋葱伯克霍尔德氏菌、金黄色葡萄球菌等;未能清除的痰病原有铜绿假单胞菌、鲍曼不动杆菌、大肠埃希氏菌、分支杆菌、金黄色葡萄球菌、溶血葡萄球菌、白色念珠菌、曲霉菌;美罗培南使用后新出现的痰病原菌有铜绿假单胞菌、嗜麦芽窄食单胞菌、肺炎克雷伯氏菌、鲍曼不动杆菌、洛菲不动杆菌、琼氏不动杆菌、脑膜脓毒性金黄杆菌、产吲哚金黄杆菌、分支杆菌、金黄色葡萄球菌、溶血葡萄球菌、白色念珠菌、近平滑念珠菌、光滑念珠菌、热带念珠菌、克柔念珠菌、曲霉菌。结论美罗培南治疗肺部感染过程中,应警惕不敏感的病原菌和新出现的病原菌。Objective To evaluate the sputum pathogen changes in meropenem treatment of pulmonary infection. Methods A retrospective study was conducted to observe the sputum pathogen changes in meropenem treatment of pulmonary infections in the pulmonary department of the first affiliated hospital of Xiamen University in 2010. Results The sputum pathogens cleared by meropenem included Pseudomonsa aeruginosa, Klebsiella pneumoniae pneumoniae, Acinetobacter baumannii, Burkholderia cepacia, Staphylococcus aureus, et al. The sputum pathogens persistent during meropenem treatment were Pseudomonsa aeruginosa, Acinetobacter baumannii, Eseherichia coli, Mycobacterium, Staphylococcus aureus, Staphylococcus haemolyticus, Candida albicans, Aspergillus. The sputum pathogens emerged during meropenem treatment were Pseudomonsa aeruginosa, Stenotrophomonas maltophilia, Klebsiella pneumoniae pneumoniae, Acinetobacter baumannii, Acinetohacter lwoffii, Acinetobacter junii, Chryseobacterium meningosepticum, Chryseobacterium indologenes, Mycobacterium, Staphylococcus aureus, Staphylococcus haemolyticus, Candida albicans, Candida parapsilosis, Candida glabrata, Candida tropicalis, Candida krusei, and Aspergillus. Conclusion During the treatment of meropenem in pulmonary infection, attentions should be paid to the insensitive pathogens and new emerging pathogens in sputum.
