类风湿关节炎患者血清IL-17、TNF-α检测的意义

摘　　要：目的探讨类风湿关节炎(rheumatoid arthritis,RA)患者血清中白细胞介素-17(IL-17)、肿瘤坏死因子-α(TNF-α)的表达情况及其滑膜病变检测的临床意义。方法应用酶联免疫吸附试验(ELISA)法对活动期RA(活动期RA组)、稳定期RA(稳定期RA组)和健康体检者(对照组)各50例外周血血清IL-17和TNF-α含量进行检测,并观察其滑膜病变情况。结果活动期RA组血清中IL-17和TNF-α含量明显高于稳定期RA组和对照组,差异均有统计学意义(P<0.01);稳定期RA组血清IL-17含量高于对照组,差异有统计学意义(P<0.05),而TNF-α含量与对照组比较,差异无统计学意义(P>0.05)。Pearson相关分析结果显示,活动期RA组血清IL-17和TNF-α水平呈显著正相关(r=0.436,P<0.05)。活动期RA组滑膜有大量淋巴细胞浸润,并出现血管增生及滑膜纤维化,稳定期RA组滑膜有少量淋巴细胞浸润及滑膜出血;对照组未发现明显病理性改变。结论 RA患者IL-17和TNF-α含量及滑膜病变特点与病情有关,联合动态监测这两种因子有助于临床观察和治疗。Objective To investigate the levels of IL-17 and TNF-α in serum and synovium pathological changes in patients with rheumatoid arthritis（RA） and its clinical significance.Methods Levels of IL-17 and TNF-α in 50 patients with RA in active phase（active phase RA group）,50 patients with RA in stable phase（stable phase RA group） and 50 normal controls（control group） were measured with ELISA,and the synovium disease was observed.Results The levels of IL-17 and TNF-α in active phase RA group were significantly higher than those in stable phase RA group and control group,and the differences were significant（P0.01）;The level of IL-17 in stable phase RA group was significantly higher than that in control group,and the difference was significant（P0.05）,but there were no difference in TNF-α level between the two groups（P0.05）.Pearson correlation analysis showed that the level of IL-17 in active phase RA group was positively related to the level of TNF-α（r=0.436,P0.05）.Synovium in active phase RA group had mass lymphocyte imbibitions and blood vessel hyperplasia and fibering,but synovium in stable phase RA group only had little lymphocyte imbibitions and bleeding,and control group had no pathological changes.Conclusion The contents of IL-17 and TNF-α and pathological changes of synovium are related to the disease changes,and combined detection of the two factors are helpful in clinical observation and treatment.
