Orem自理模式应用于慢性精神分裂症患者的护理

摘　　要：目的研究Orem自理模式应用于慢性精神分裂症患者的护理效果。方法将120例住院慢性精神分裂症患者按住院号的单双号随机分为实验组60例和对照组60例,在药物治疗的基础上,对照组按传统进行常规护理,实验组则根据病人病情发展阶段自理能力的不同,按Orem自理模式给予完全补偿、部分补偿和支持-教育。两组分别在入组时、入组8周末、出院时采用日常生活能力量表(ADL)、护士用住院病人观察量表(NOSIE)进行评定,出院后随访半年。结果实验组在社会能力、社会兴趣、个人卫生等积极因素的提高,退缩、激惹、抑郁等消极因素的恢复方面显著优于对照组(p<0.05或p<0.01)。躯体生活自理及工具性自理能力评分显著高于对照组及干预前(p<0.05或p<0.01)。随访半年再住院率实验组为20%;对照组为30%。结论应用Orem自理模式对慢性精神分裂症患者进行护理干预,能发掘患者自身护理潜能,提高其自我护理能力,使患者由被动护理变为主动参与自身护理,从而提高患者日常生活质量,促进其社会功能恢复,同时也减轻了家庭和社会的负担。Objective To study the Orem self-care model used in the care of patients with chronic schizophrenia effect. Methods 120 patients hospitalized with chronic schizophrenia according to hospital number of odd and even numbers were randomly divided into experimental group（60 cases） and control group（60 cases）,With drug treatment,based on the control group according to the traditional routine care,the experimental group are based on the patient self-care ability of the disease in different stages of ill-condition development,according to Orem self-care model to give full compensation,partial compensation,and support together education.Two groups,respectively,Before intervention.into the group of 8th weekend,at discharge using Activity of Daily Living Scale（ADL）,Nurses＇ Observation Scale for In-patient Evaluation（NOSIE） to be assessed,followed up six months after discharge. Results In the experimental group,The positive factors of social ability,social interest and personal hygiene etc showed improvement,the negative factors of withdrawal,irritability,depression etc was recovery significantly better than the control group（p0.05 or p0.01）.Physical life self-care and instrumental self-care ability score were significantly higher,and before the intervention（p0.05 or p0.01）.follow-up six months the experimental group re-hospitalization rate was 20%;the control group was 30%. Conclusion Orem self-care model of nursing intervention in patients with chronic schizophrenia,To explore the potential of the patient＇s own care,improve their self-care ability,so that patient self care abilities could be from passive to active participation,thereby improving the quality of life of patients and promote their social function recovery,but also reduce the burden on family and society.
