摘  要:目的:探讨坎离颗粒对腹主动脉缩窄所致心衰大鼠左室血流动力学的影响及可能作用机制。方法:腹主动脉不完全结扎法制备大鼠心衰模型。用坎离颗粒(生药12 g.kg-1)ig该模型24周,阳性对照组以开搏通(3.375 mg.kg-1)ig 24周。24周后,压力传感器记录左室血流动力学改变;测定左心室及全心质量指数(LVWI,HWI);苦味酸天狼猩红染色测量心肌胶原含量。结果:坎离颗粒可提高慢性心衰大鼠的左室收缩内压(LVSP),降低左心室舒张末压(LVEDP),改善左室内压最大变化速率(±LVdp/dtmax)(P<0.01),从而有效地改善了心衰大鼠的血流动力学;降低胶原容积分数(CVF)、心脏指数(P<0.05)和左室质量指数(LVMI)(P<0.01),升高I/Ⅲ型胶原比值(Ratio of type I/1I)(P<0.05);部分指标优于开搏通组。结论:坎离颗粒可改善压力超负荷心衰大鼠的血流动力学、抑制其心室重构,其机制可能与其抗心肌纤维化有关。Objective:To investigate the effect and its mechanism of Kanli granule on heart failure hemodynamics in chronic heart failure by partial coarctation of abdominal aorta in rats.Method:The chronic heart failure(CHF) model was induced by partial coarctation of abdominal aorta in rats.The CHF rats were feeded with Kanli granule(unprepaired herbal medicine 12 g·kg-1·d-1)for 24 weeks,and the captopril group(3.375 medicine given Kanli granule 12 g·kg-1·d-1)for 24 weeks.After 24 weeks,cardiac hemodynamics parameters,left ventricular mass index(LVMI),Cardiac mass index(CMI),left ventricular systolic pressure(LVSP),left ventricular diastolic pressure(LVDP),left ventricular end-diastolic pressure(LVEDP),collagen volume fraction(CVF),collagenⅠ/Ⅲ ratio were measured pathologically.Myocardial cell and interstitial collagen were observed by HE and VG stain.Result:Kanli granule availably improved cardiac hemodynamics parameters including LVSP,LVEDP,±LVdp/dtmax,decreased CVF and CMI(P0.05) including LVMI(P0.01),meanwhile increased Ratio of type I/1I)(P0.05).Conclusion: Kanli granule can availably improved cardiac hemodynamics parameters of rats with CHF,regulate over-degradation of myocardial collagen and prevent ventricle expanding which are related to relieving the myocardial extracellular matrix remodeling of CHF.
