摘  要:目的评价老年急性心肌梗死(AMI)患者颈动脉粥样硬化斑块的超声回声特征及其狭窄程度。方法选择AMI患者325例,按年龄分为3组,<70岁组63例,70~79岁组140例和≥80岁组122例。3组均行彩色多普勒超声检查,并进行分析。结果与<70岁组比较,70~79岁组和≥80岁组患者无颈动脉斑块的比例明显降低,斑块数≥3个、斑块厚度≥0.19 cm及稳定斑块的比例增高,差异有统计学意义(P<0.05)。3组患者心肌梗死后发生脑梗死的比例均明显高于脑梗死后发生心肌梗死的比例。结论多数老年AMI患者的颈动脉为轻度狭窄和不狭窄,多为不稳定斑块。斑块的超声回声特征较斑块的数量及其狭窄程度更重要。Objective Assessment of echo characteristics of carotid atherosclerotic plaque(CASP) and its stenosis cases with acute myocardial infarction(AMI) in different ages by using Doppler ultrasound. Methods 325 AMI cases were selected and divided into 3 groups according to their ages. Group Ⅱ included 63 cases aged less than 70 years. Group Ⅲ included 140 cases aged between 70--79 years and group Ⅲ included 122 cases aged more than 80 years. Results The ratios with-out CASP were lower;for the number and thickness of CASP nespectively more than 3 and 0. 19 cm, its stability in group Ⅱ and Ⅲ was higher than that in group Ⅰ (P〈0.05). The ratio of AMI ca- ses accompanied with CI later was higher than those CI cases accompanied with MI later. Conclusions Most carotid atherosclerosis in elderly cases with AMI were mild and without stenosis. The CASP in these cases were mostly unstable plaques. We thought that unstable CASP,mild CS and no stenosis happened to most patients with MI. The echo characteristics of CASP were more important than total number of plaques and degree of CS for patients with AMI. We should pay more attention to the unstability of CASP and actively intervening treatment should be taken earlier.
