学龄儿童超重肥胖与进食障碍的病例对照研究

摘　　要：目的探讨超重肥胖儿童进食障碍状况及相关心理行为特征,为预防肥胖和进食障碍的发生提供参考依据。方法以1∶2配对的方法,采用进食障碍问卷儿童版(Eating Disorders Inventory of Children,EDI-C),对上海市某区6所小学二～四年级186名超重肥胖儿童以及372名正常体重儿童进行调查,评价超重肥胖与发生进食障碍的相关性。结果超重肥胖组求瘦倾向、形体不满、自信心不足和内感受缺陷分量表得分超过正常值的人数比例显著高于体重正常组(P<0.01);体重正常组的贪食分量表得分超出正常值的人数比例高于超重肥胖组。超重肥胖组中,女生更易患贪食,但不同年龄组女生量表得分超过正常值的比例差异无统计学意义。结论儿童肥胖与形体不满及求瘦倾向等进食障碍的发生有一定相关性。需要对进食障碍儿童进行全面干预和适当行为矫正。Objective To explore the eating disorder status among school children, and to provide references for preventing obesity and eating disorder. Methods According to the BMI classification standards, 186 obese children and 372 health children of 2 to 4 grades in 6 primary schools in Shanghai were investigated with Eating Disorders Inventory of Children （ EDI-C ） by 1 ： 2 matched case-control study. Results More children whose scores of drive for thinness and body dissatisfactory subscales over typical clinic and elevated clinic standards in case group than that in control group ; More children whose scores of low self-esteem and interceptive deficits subscales only over elevated clinic standards in case group than that in control group; While in terms of bulimia subscale, more children whose scores over normal standards in control group than that in case group. The negative attitudes to body satisfaction and drive for thinness among case group were significantly higher than control group（ P〈0.05 ）. The attitudes and behaviors to bulimia were more serious among girls, but statistical analyses revealed no significant age differences in the EDI subscales. Conclusion Eating disorders and related symptoms, such as body dissatisfaction and drive for thinness, are related to children obesity. Since its continuous effect on adulthood, there is a need for complete prevention and proper interference.
