Rockwood Ⅲ～Ⅴ型肩锁关节脱位两种手术方式的比较研究

摘　　要：目的比较双Endobutton钢板与锁骨钩钢板治疗RockwoodⅢ～Ⅴ型肩锁关节脱位的临床疗效。方法对采用锁骨钩钢板(24例)和双Endobutton钢板(18例)治疗的42例RockwoodⅢ～Ⅴ型肩锁关节脱位患者进行了8～18个月的随访,比较两组术后肩痛的发生率、术后肩关节外展的活动度和临床疗效。结果术后肩痛发生率Endobutton组(5.5%)明显低于锁骨钩钢板组(45.8%)。术后肩关节外展活动度Endobutton组优于锁骨钩钢板组。临床疗效按Karlsson标准评定,Endobutton组优15(83.3%)例,良3(16.7%)例,优良率100%;锁骨钩钢板组优15(62.5%)例,良8(33.3%)例,差1(4.1%)例,优良率95.8%;两组比较,差异无统计学意义(P>0.05),但Endobutton组优秀率高于锁骨钩钢板组(P<0.05)。结论双Endobutton钢板治疗RockwoodⅢ～Ⅴ型肩锁关节脱位的效果与锁骨钩钢板相当,但具有对肩关节功能影响少、术后肩痛发生率低、内固定存留不必取出等优点。Objective To evaluate and compare the outcomes of double Endobutton and clavicular hook plate for the treatment of Rockwood type Ⅲ- V acromioclavicular ioint dislocation. Methods We have conducted two groups of patients, one for double Endobutton（ 18 patients） and another for clavicular hook plate（24 patients） ,for 10 - 18 month＇s follow - up. To compare shoulder pain incidence and shoulder motion difference between two groups. Results Shoulder pain incidence was significantly different in which double Endobutton group was 5.5% and clavicular hook plate group was 45.8%. The double Endobutton group showed a better shoulder motion than the e]avicalar hook plate group. According to Karlsson stand- ard, the clinical effect in two groups was not significant statistically （ P 〉 0.05 ） , for Endobutton group was 83.3 % good （ 15 patients）, 16.7 % better （3 patients） , excellence rate was 100% and clavicular hook plate group was 62.5 % gond （ 15 patients） ,33.3% better （8 patients） ,4. I% poor（1 patient） ,excellence rate was 95. 8% , ho,gever the excellence rate was significant high of the fomer group （ P 〈 0.05 ）. Conclusion The clinical effect of double Endobutton for treating with Rockwood type Ⅲ- V acromioclavieularioint dislocation is similar to clavicular hook plate. But the former is better for joint function, lower incidence of shoulder pain and needn＇t to remove the internal fixation retained than the later.
