摘 要:[目的]分析厄贝沙坦敏感组与不敏感组高血压患者的代谢差异,探索厄贝沙坦产生不同降压作用的代谢组学物质基础。[方法]遴选单独服用厄贝沙坦敏感的和不敏感的高血压患者各30例,应用GC-TOF/MS测定厄贝沙坦敏感组、不敏感组高血压病人、以及健康志愿者血清内小分子,采用主成分分析(PCA)和偏最小二乘-判别分析(PLS-DA)法分析高血压病人与健康志愿者、厄贝沙坦敏感组与不敏感组的之间的代谢差异并鉴定差异化合物。[结果]与健康志愿者相比,高血压病人血清中大部分脂类(除单硬脂酸甘油酯)、游离脂肪酸较高;氨基酸中除苯丙氨酸、丙氨酸较高外,丝氨酸、色氨酸、苏氨酸水平较低。厄贝沙坦敏感组血清中缬氨酸、谷氨酰胺、甘氨酸、核糖醇、尿素、哌啶酸较高;而胆固醇、丙酮酸较低。[结论]高血压人群存在糖代谢、脂代谢、氨基酸代谢的异常,而厄贝沙坦敏感组与不敏感组的代谢差异提示与厄贝沙坦降压敏感性有关。[Objective]To analyze metabolic differences between irbesartan sensitive group and non-sensitive group of hypertensive patients,explore the antihypertensive effect of irbesartan in different patients from its material basis of metabolomics.[Methods]Selected patients taking irbesartan alone and divide them into irbesartan sensitive and non-sensitive group,30 cases in each group.Used GC-TOF/MS to test the small serum molecule in sensitive group,non-sensitive group and healthy volunteers,and used principal component analysis(PCA)and partial least squares-discriminant analysis(PLS-DA)to analyze metabolic differences and identify different compounds among hypertensive patients,healthy volunteers,irbesartan sensitive and non-sensitive group.[Results]Compared with healthy volunteers,the majority of serum lipids(except monostearate)and free fatty acid in hypertensive patients were higher,the amino acids except phenylalanine,alanine which were higher than control group,the rest like serine,tryptophan,threonine were lower than the control group.Irbesartan sensitive group had higher valine,glutamine,glycine,ribose alcohol,urea,piperidine acid in serum,but lower cholesterol,pyruvate.[Conclusion]Hypertensive patients exist abnormal sugar metabolism,lipid metabolism,and amino acids metabolism,and the metabolic differences between irbesartan sensitive group and non-sensitive group suggest that it is associated with Irbesartan's antihypertension sensitivity.
