摘  要:[目的]探讨机械通气治疗急性护理窘迫综合症的疗效观察。[方法]将某院2005年12月-2010年12月收治的60例急性呼吸窘迫综合症的患者随机分为观察组和对照组,每组30例,两组均采用基础治疗,对照组采用传统的机械通气治疗,观察组采用肺保护性通气,比较两组患者的治疗效果、带机时间、呼吸机相关性肺炎(VAP)发生率、病死率。[结果]两组患者的动脉血氧分压及动脉二氧化碳分压均较治疗前有明显改善(P﹤0.05),且两组之间差异无统计学意义(P﹥0.05)。观察组的带机时间、VAP发生率及病死率分别为(5.2±1.5)d、20.0%(6/30)、6.7%(2/30)显著优于对照组的(7.5±2.3)d、33.0%(10/30)、20.0%(6/30),P﹤0.05。[结论]机械通气能够迅速缓解急性呼吸衰竭患者的缺氧症状,肺保护性通气治疗优于传统机械通气治疗,有利于减少并发症,降低死亡率。[Objective]To explore the clinical effect of mechanical ventilation treatment on acute respiratory distress syndrome(ADRS).[Methods]From December 2005 to December 2010,60 cases with ARDS were randomly divided into two control group and experience group.30 cases of control group were treated with traditionary mechanical ventilation and basic therapy,while 30 cases of experience group were performed lung protective ventilation.Therapeutic efficacy,mechanical ventilation use time,incidence rate of ventilator-associated pneumonia(VAP)and case fatality were compared between two groups.[Results]PaO2 and PaCO2 of post-treatment were significantly improved than that of pre-treatment in two groups(P﹤0.05),without significant difference between two groups(P﹥0.05).As for the experience group,mechanical ventilation use time,incidence rate of VAP and case fatality were 5.2±1.5d,20.0%(6/30),6.7%(2/30)respectively,of which were 7.5±2.3d,33.0%(10/30)and 20.0%(6/30)in control group(P﹤0.05).[Conclusion]Mechanical ventilation for ADRS can relieve anoxia symptoms quickly,and lung protective ventilation is superior to traditionary mechanical ventilation because of lower incidence rate of complication and death rate.
