摘  要:目的观察慢性阻塞性肺疾病(Chronic obstructive pulmonary disease,COPD)患者纤维蛋白原(FG)、D-二聚体与血气分析的变化,并探讨其相关性及临床意义。方法对60例确诊为COPD患者进行前瞻性研究。将患者分为两组[急性发作期(n=30)和临床缓解期(n=30)],并选取正常对照组30例。采用凝固法及liates法检测纤维蛋白原和D-二聚体,同时使用血气分析仪测定血气分析。结果 COPD患者急性发作期FG、D-二聚体含量高于临床缓解期(P<0.01),两期患者的FG、D-二聚体含量又高于正常对照组(P<0.05)。且FG、D-二聚体的增高与PaCO2呈正相关(P<0.01),与PaO2呈负相关(P<0.01)。结论凝血功能的检测对于COPD的发展及治疗具有重要的临床意义。Objective To study the changes of D-dimer,plasma fibrinogen(FG) and blood gas analysis in patients with chronic obstructive pulmonary disease(COPD) as well as to investigate their correlation and clinical significance.Methods 60 patients proven with COPD participated in this prospective study,which was divided into active stage group(n=30) and remittent stage group(n=30) and 30 healthy individuals were chosen as control group.A Freezing method and a Liates' method were used to measure FG levels and D-dimer level,respectively.Blood gas analyzer was also used to measure blood gas analysis.Results Compared to patients with COPD at remittent stage,the D-dimer and plasma fibrinogen levels were elevated in patients at active stage,there were statistically significant differences(P0.01).The D-dimer and plasma fibrinogen levels were significantly higher in 60 exacerbated patients than in 30 controls (P0.05).The elevated D-dimer level and plasma fibrinogen levels were negatively associated with PaO2(P0.01),but they were positively associated with PaCO2(P0.01).Conclusion The coagulation function test may benefit clinical diagnosis and treatment for COPD.
